Clinical significance of positive for high risk HPV and negative cytology in cervical cancer screening

Abstract

Objective: The aim of this study was to investigate the appropriate management of patients with negative cytology, but positive high risk HPV testing.

Methods: Medical records were reviewed for patients who had cervical cancer screening by both Pap smear and HPV DNA testing between January 2003 and December 2006. Among 3,158 patients who had both Pap smears and HPV DNA tests, 516 patients had negative cytology with positive high risk HPV. After exclusion of 237 cases in which follow-up data were not available, the remaining 279 cases were reviewed.

Results: After a mean follow-up period of 12.3¡¾8.6 months, 250 cases (89.6%) had consecutive normal cytologies by biannual Pap smears and 29 cases (10.4%) had abnormal cytologies. The results of colposcopy-directed punch biopsy for these abnormal cytologies were 18 cases of chronic cervicitis (6.5%), 2 case of flat condyloma (0.7%), 5 case of CIN I (1.8%), 3 cases of CIN III (1.1%), and 1 case of verrucous hemangioma (0.4%).

Conclusion: Our follow-up cytology and biopsy results suggest that frequent exams at less than 12 month intervals is unnecessary for women with positive for high risk HPV and negative cytology.
