Abstract

Contribution evasion of the National Health Insurance (NHI) is a critical problem in Korea which has a social insurance system mostly funded by compulsory contribution. The low-income and healthy population tends to evade NHI contribution and become ineligible to receive benefits. The objective of the present study was to investigate factors associated with the evasion of NHI contribution among low-income households and general households using the Korea Welfare Panel Study data from 2008 to 2012. Among those eligible for NHI, we defined those with income lower than 60 percent of median household income level as low-income households. Random effect logistic models were used for statistical analysis. The results showed that low-income households more often evaded NHI contribution for a longer period of time than general households. The contribution evasion was associated with young breadwinner, living in urban areas, the self-employed insured, less wealthy households, and low disease needs in both groups, and the odds of evasion were higher among low-income households than general households. In conclusion, it is necessary to improve the contribution rating system including contribution reduction on low-income households in order to keep them eligible to receive the benefits of the NHI.
