Abstract

¸ñ Àû : Á¤À§ÀûüºÎ¹æ»ç¼±Ä¡·á(Stereotatic body radiation therapy, SBRT)´Â ¿ø¹ß¼º Á¾¾ç°ú ÀüÀ̼º Á¾¾çÀÇ Ä¡·á¹ýÀ¸·Î ÀÌ¿ëµÇ¾îÁø´Ù. SBRT´Â ³ôÀº ¼±·®À» Àü´ÞÇϱ⠶§¹®¿¡ MU(Monitor unit)ÀÇ Áõ°¡·Î Ä¡·á½Ã°£ÀÌ ±æ¾îÁö°í Ä¡·á°èȹ¿ëÀû(Planning Target Volume, PTV) ÁÖº¯ÀÇ Á¤»óÀå±â¸¦ È¿°úÀûÀ¸·Î º¸È£ÇÒ ¼ö ÀÖ´Â Ä¡·á°èȹÀÌ ÇÊ¿äÇÏ´Ù. °á °ú : PTV¿¡ ´ëÇÑ Ã³¹æ¼±·®Áö¼ö(Conformity Index, CI), ¼±·®±ÕÁúÁö¼ö(Homogeneity index, HI), ó¹æ¼±·®Æ÷ÇÔÁö¼ö(Paddick¡¯s Conformity Index, PCI)ÀÇ Æò±Õ°ªÀº VMAT¿¡¼­ °¢°¢ 1.006¡¾0.028, 1.098¡¾0.016, 1.132¡¾0.084, CRT¿¡¼­ 1.381¡¾0.419 1.136¡¾0.042, 1.534¡¾0.465·Î Æò°¡µÇ¾ú´Ù. Á¤»óÀå±â¿¡ ´ëÇÑ ¼±·®Àº CRT¿¡¼­ VMAT º¸´Ù ¾à 1.8¹è ³ôÀº ¼±·®À¸·Î Æò°¡µÇ¾ú´Ù. Àüü MU°ªÀº VMAT¿¡¼­ ¾à 1.3¹è ³ô°Ô Æò°¡µÇ¾ú°í VMAT°ú CRT µÎ Ä¡·á°èȹÀÇ Æò±Õ Beam on timeÀº °¢°¢ 6.8ºÐ, 21.3ºÐÀ¸·Î Æò°¡µÇ¾ú´Ù.

°á ·Ð : ´ÙÁßÇ¥Àû °£¾ÏÀÇ Á¤À§ÀûüºÎ¹æ»ç¼±Ä¡·á¿¡¼­ FFF BeamÀ» ÀÌ¿ëÇÑ VMATÀ» Àû¿ëÇÏ¸é ¼±·® °ãħ ¾øÀÌ ´ÙÁßÇ¥ÀûÀ» ÇÑ ¹ø¿¡ Ä¡·áÇÒ ¼ö ÀÖÀ¸¸ç PTVÀÇ ¼±·®Æ÷ÇÔÀ» ¸¸Á·Çϸ鼭 ÁÖÀ§ÀÇ Á¤»óÀå±â¸¦ ´õ È¿°úÀûÀ¸·Î º¸È£ÇÒ ¼ö ÀÖ´Â Ä¡·á°èȹÀÌ °¡´ÉÇÏ´Ù. ¶ÇÇÑ FFF BeamÀÇ ³ôÀº ¼±·®·ü(Dose rate)À» ÀÌ¿ëÇÏ¿© Ä¡·á½Ã°£À» ´ÜÃà½ÃÄÑ Ä¡·á Áß ¹ß»ýÇÒ ¼ö ÀÖ´Â ¿ÀÂ÷¸¦ °¨¼Ò½Ãų ¼ö ÀÖ´Ù.
Purpose : Stereotactic body radiation therapy (SBRT) has proved its efficacy in several patient populations with primary and metastatic limited tumors. Because SBRT prescription is high dose level than Conventional radiation therapy. SBRT plan is necessary for effective Organ at risk (OAR) protection and sufficient Planning target volume (PTV) dose coverage. In particular, multi-target cases may result excessive doses to OAR and hot spot due to dose overlap. This study evaluate usefulness of Volumetric modulated arc therapy (VMAT) in dosimetric and technical considerations using Flattening filter free (FFF) beam.

Materials and Methods : The treatment plans for five patients, being treated on TrueBeam STx(VarianTM, USA) with VMAT
using 10MV FFF beam and Standard conformal radiotherapy (CRT) using 15MV Flattening filter (FF) beam. PTV, liver,
duodenum, bowel, spinal cord, esophagus, stomach dose were evaluated using the dose volume histogram(DVH). Conformity index(CI), homogeneity index(HI), Paddick¡¯s index(PCI) for the PTV was assessed. Total Monitor unit (MU) and beam on time was assessed.

Results : Average value of CI, HI and PCI for PTV was 1.381¡¾0.028, 1.096¡¾0.016, 0.944¡¾0.473 in VMAT and 1.381¡¾0.042, 1.136¡¾0.042, 1.534¡¾0.465 in CRT respectively. OAR dose in CRT plans evaluated 1.8 times higher than VMAT. Total MU in VMAT evaluated 1.3 times increase than CRT. Average beam on time was 6.8 minute in VMAT and 21.3 minute in CRT.

Conclusion : VMAT for SBRT in multi-target liver cancer using FFF beam is effective treatment techniqe in dosimetric and technical considerations. VMAT decrease intra-fraction error due to treatment time shortening using high dose rate of FFF beam.
