Abstract

연구목적: 과거에 비해 점점 전 세계의 많은 나라들에 있어서 소득수준이 증가하고 있으며, 이와 더불어 평균수명은 증가하고 영유아 사망률은 감소하고 있는 추세이다. 그러나 이러한 지표들에 있어서 여전히 각국간에 커다란 격차가 있음은 물론, 그 차이가 점점 더 심해지고 있는 것이 사실이다. 세계보건기구(WHO)에서는 “World Health Report”라는 연간 보고서를 통해 최근까지 세계 각국의 평균수명이나 영아사망률과 같은 건강관련 통계 및 의료비지출과 같은 경제통계를 발표하였으며, 본 연구에서는 이러한 자료를 이용하여 WHO 가맹국을 대상으로 기대여명 및 영아사망률에 어떠한 요인들이 영향을 미치는지에 관하여 분석하는 것을 연구의 목적으로 하였다.

연구방법: 건강지표와 의료비 지출의 가장 최근 자료인 “World Health Report 2005”를 이용하여 WHO 가맹국 192개국을 분석대상으로 하였다. 분석대상의 출생시 평균 기대여명은 65.5세였고, 5세 이하 영아사망률은 1000명당 61.8명이었으나 각국간 3.0명에서 283.0명으로 커다란 편차를 보였다. 국내총생산(GDP) 대비 의료비지출은 평균 6.2%였으며, 구매력지수로 환산한 일인당 국민소득(GNI)은 9,855달러(US)였으며 620달러에서 61,220달러까지의 편차를 보였다.

연구결과: 요인분석 결과, 설명변수들이 공공의료비지출, 민간의료비지출, 공적 지원의 세 요인으로 분류되었다. 추가로 실시된 로그·리니어 다중회귀분석(log-linear multiple regression analysis)을 통해 공공의료비지출과 공적 지원이 기대여명 및 영아사망률과 통계적으로 유의한 양의 관계를 갖는 것을 알 수 있었다. 한편, 민간의료비지출은 기대여명에는 다소 유의한 관계가 있었지만 영아사망률과는 관계가 없는 것으로 확인되었다. 본 연구의 제한점으로는 건강수준의 국제비교를 함에 있어 경제적 요인 외에 다른 사회적, 정치적 요인들을 고려하지 못했다는 점을 들 수 있으나 다른 많은 연구에서도 경제적인 요인을 가장 중요한 요소로 파악하고 있다는 점을 지적할 수 있으며, 가능하다면 시간의 추이에 따른 변화를 보기 위해 패널데이터를 이용하는 것이 바람직하겠으나 WHO 자료의 한계로 인하여 가능하지 않았다.

결론: 본 연구의 성과는 건강수준의 향상을 위해서 공공의료비지출이나 공적 지원과 같은 공공부문의 역할이 기초적인 역할을 한다는 점을 알아냈다는 데에 있다고 하겠다.

Objectives: Life expectancy has extended and infant mortality has improved with the development of medical sciences and health care technology than ever before around the world. From these indicators, however, it is also true that the gap among these countries has even widened.

Methods: Analyses were made following two steps: First, a multivariate factor analysis was conducted to discover the correlation between both life expectancy and infant mortality and each explanatory variable used in this study. The analysis was expected to discover characteristics of explanatory variables on dependant variables. Second, a multiple regression analysis with exploratory factor analysis was conducted to identify key correlates with determinants of life expectancy and infant mortality.

Results: By the factor analysis, three clusters of variables appeared representing private expenditure on health, public expenditure on health, and social support each. And, an exploratory factor analysis found that public expenditure on health and social support were related with both life expectancy and infant mortality, whereas private expenditure on health was somewhat related with life expectancy but not with infant mortality.

Conclusion: Encouraging findings from the study were that public sector such as public expenditure on health or social support has an important role in improving health outcomes. Health policy makers should pay attention to the results from this study and take action for the betterment of our nation.
