Abstract
I. Introduction
Knowledge of the morphology of the root canal systems is required for successful root canal treatment. Mandibular premolars have a variety of root canal configurations. One of the most challenging configurations for diagnosis and treatment is found in the mandibular premolars with a C-shaped canal system. The following case reports present two cases of mandibular premolars with C-shaped canals.
II. Clinical case
1. Sex/Age: F/12
2. Chief Complaint (C.C): Continuing pain of #34
3. Past Dental History (PDH): N/S
4. Present Illness (P.I): #34 Dens evaginatus, Per(+), Pal(+), EPT(-), Cold/Hot(-), Periapical radiolucency
5. Impression: Pulp necrosis with symptomatic apical abscess on #34
6. Treatment plan: #34 Endodontic treatment

2. Chief Complaint (C.C): Continuing pain of #35
3. Past Dental History (PDH): N/S
4. Present Illness (P.I): #35 Dens evaginatus, Per (+), Cold/Hot(-), Periapical radiolucency, Sinus tract
5. Impression: Pulp necrosis with symptomatic apical abscess on #35
6. Treatment plan: #35 Endodontic treatment
III. Conclusion
The mandibular premolar teeth can present with extremely complex root and canal system morphology, and if not considered during treatment can lead to difficulties when performing root canal treatment. Careful examination of preoperative radiographs and thorough inspection of internal anatomy of teeth are required for successful endodontic treatment.
