Abstract
Diagnosis is a first step of endodontic treatment. In every case, we wonder if we have to start endodontic treatment or not. There are many difference of the diagnostic accuracy following the technique of diagnosis and interpretation. And we are very confusing how to interpret the result of diagnosis. Sometimes in spite of following the guideline of diagnosis in textbook, we meet different situation. In some case, even if the result of vitality test was negative but when I opened pulp chamber, I saw bleeding from the opening of chamber. On the other hand, result of vitality test was positive but I found some canal was pulp necrosis.
In retreatment case, we are in the state of chaos because most of retreatment cases have no symptom. Patient doesn¡¯t have pain except apical radiolucency on periapial film. This means we have to decide the future of the endodontic treatment.
In clinical test, decision of endodontic treatment depend on vitality test. Usually we use cold test and electronic vitality test but the accuracy of diagnosis is not 100% in spite of using both techniques. We have to decide to relief a pain of patient in uncertain diagnosis. If we think of the condition of patient, it is wrong that we order the patient have to wait without any promise of pain free.
Today I want to discuss how to control this unpredictable case.
