Abstract

의료의 전문적 특성으로 인해 의사들에게는 고유의 직업 윤리가 요구되며, 임상 현장에서 접하게 되는 다양한 문제들의 해결에 개별 의사의 윤리의식은 중요하게 작용할 수 밖에 없다. 그러므로 의과대학생 때부터 올바른 윤리적 판단을 내릴 수 있는 훈련이 되어있어야 하고, 의료윤리 교육은 이를 위한 도구를 제공하는 역할을 하여야 한다. 본 연구는 사회적 직관주의 이론을 바탕으로 하여 의학전문대학원생들과 전공의들의 도덕적 직관의 범주를 측정하고, 의사들의 직업적 책임감의 정도를 파악하고자 하였다. 설문 조사는 서울의 한 의학전문대학원 학생들과 대학병원 전공의를 대상으로 하였다. Moral Foundations Questionnaire를 번안한 설문지를 통해 ‘배려’, ‘공정성’, ‘충성심’, ‘권위’, ‘고귀함’의 다섯 영역으로 구분된 도덕적 직관을 확인하였으며, 여기에 의사의 직업윤리에 관련된 내용을 추가하였다. 설문 응답률은 75.5%였으며 응답자 중 의학전문대학원생은 340명, 전공의는 30명이었다. 환자를 진료하는 데 있어 학생과 전공의 모두 자율성 존중의 원칙이나 정의보다 ‘선행/해를 가하지 않는 것’을 가장 우선적인 윤리원칙으로 선택하였다. 도덕적 신념에 대한 인식은 배려>공정성>충성심 및 고귀함>권위 순으로 두 군에서 동일하였다. 전반적으로 전공의에 비해 학생들의 평균 점수가 상대적으로 높았으며, 각 항목의 하위 질문들 중에는 두 집단 간에 유의미한 차이를 보이는 것이 있었다. 의사들의 직업윤리와 도덕적 신념들 사이의 관련성 확인을 위하여 로지스틱 회귀분석을 시행한 결과 정의를 기본 윤리원칙으로 두는 경우다른 사람들에 비해 본인의 위험을 감수하고서라도 환자를 치료해야 할 직업적 의무가 있다고 생각하는 것으로 나타났다(OR, 2.27; 95% CI, 1.23~4.18; p=0.008). 사회적 직관주의는 도덕적 직관의 범주들이 한국 사회에서 어떻게 기능하는 지를 파악하는 데 사용될 수 있을 뿐만 아니라, 의료윤리 교육과정에서 학생들이 피드백을 통해 스스로의 직관에 대해 회고해보는 계기를 제공할 수 있을 것이다.

This study was designed to determine whether or not there are differences between the moral intuitions of medical students and those of residents in clinical fields. A total of 370 of 490 (75.5%) students (n=340) and residents (n=30) at one university in South Korea responded to a survey consisting of the Moral Foundations Questionnaire (MFQ30) and additional items on moral judgments concerning the duty to care during pandemic influenza. Both groups selected beneficence/non-maleficence (as opposed to autonomy or justice) as the primary ethical principle(s) in medical practice. The mean scores on a 0~5 scale for the five moral foundations for the students were not significantly different from those of the residents (harm; 3.6 vs. 3.4, fairness; 3.5 vs. 3.3, loyalty; 3.1 vs. 3.0, authority; 2.9 vs. 2.8, sanctity; 3.1 vs. 3.0, respectively). However, there was considerable variability in individual items forming the scales for the fairness foundation. Interestingly, when respondents chose justice first among ethical principles, they felt that physicians had a duty to treat patients with influenza, even if in doing so they placed themselves or their families at risk (OR 2.27, 95% CI 1.23~4.18). Our data suggest that social intuitionism may be useful for explaining differences in Korean physicians’ moral judgments and also that it may be an effective tool for feedback in medical ethics education.
