Abstract
¸ñÀû: Helicobacter pylori (H. pylori) °¨¿°ÀÌ À§Ã༺ À§¿°, Àå»óÇÇ È»ýÀ» À¯¹ß½ÃÄÑ À§ ¼±Á¾°ú À§¾ÏÀ¸·Î ÁøÇàµÉ ¼ö ÀÖ´Ù´Â »ç½ÇÀº Àß ¾Ë·ÁÁ® ÀÖ´Ù. º» ¿¬±¸´Â ³»½Ã°æ Á¡¸·ÇÏ ¹Ú¸®¼úÀ» ½ÃÇàÇÑ À§ ¼±Á¾ ¶Ç´Â Á¶±â À§¾Ï ȯÀÚ¿¡¼ ´ÜÀÏ º´º¯ÀÏ ¶§¿Í µ¿½Ã ´Ù¹ß¼º º´º¯ÀÏ ¶§ H. pylori °¨¿° ¿©ºÎ¿Í Á¡¸· À§Ãà, Àå»óÇÇ È»ý µîÀÇ ±âÀú Á¶Á÷ÇÐÀû Â÷ÀÌ°¡ ÀÖ´ÂÁö¸¦ º¸°íÀÚ ÇÏ¿´´Ù.
´ë»ó ¹× ¹æ¹ý: 2005³â 4¿ùºÎÅÍ 2007³â 11¿ù±îÁö ³»½Ã°æ Á¡¸·ÇÏ ¹Ú¸®¼úÀ» ½ÃÇà¹ÞÀº 682¸íÀÇ À§ ¼±Á¾ ¶Ç´Â Á¶±â À§¾Ï ȯÀÚ°¡ ÀüÇâÀûÀ¸·Î µî·ÏµÇ¾ú´Ù. ÀýÁ¦¼ú ½ÃÇà½Ã ±Þ¼Ó ¿ä¼ÒºÐÇØÈ¿¼Ò °Ë»ç¿Í ÁÖÀ§ Á¤»ó Á¶Á÷ÀÇ Á¶Á÷°Ë»ç¸¦ ÅëÇؼ H. pylori °¨¿° ¿©ºÎ¿Í º´¸®Á¶Á÷ÇÐÀû Â÷À̸¦ °üÂûÇÏ¿´´Ù.
°á°ú: ÃÑ 682¸íÀÇ È¯ÀÚ Áß 545¸í(79.9%)ÀÇ È¯ÀÚ´Â ´ÜÀÏ º´º¯À», 137¸í(20.1%)ÀÇ È¯ÀÚ´Â µÎ °³ ÀÌ»óÀÇ º´º¯À» °¡Áö°í ÀÖ¾ú´Ù. µ¿½Ã ´Ù¹ß¼º º´º¯À» °¡Áø ȯÀÚÀÇ ³ªÀÌ(64¼¼)´Â ´ÜÀÏ º´º¯À» °¡Áø ȯÀÚÀÇ ³ªÀÌ(61¼¼)º¸´Ù À¯ÀÇÇÏ°Ô ¸¹¾ÒÀ¸¸ç(P=0.037), ¼ºº°°ú H. pylori °¨¿° ¿©ºÎ´Â µÎ ±º°£¿¡ Â÷ÀÌ°¡ ¾ø¾ú´Ù. Á¶Á÷ÇÐÀû °Ë»ç¿¡¼ üºÎÀÇ Á¡¸· À§Ãà ¹× ÀüÁ¤ºÎ¿Í üºÎÀÇ Àå»óÇÇ È»ýÀÌ ´Ù¹ß¼º º´º¯°ú ¿¬°ü¼ºÀÌ ³ô¾Ò°í, ´Ùº¯·® ºÐ¼®¿¡¼´Â °í·É°ú üºÎÀÇ Àå»óÇÇ È»ýÀÌ ´Ù¹ß¼º º´º¯°ú À¯ÀÇÇÑ »ó°ü°ü°è°¡ ÀÖ¾ú´Ù.
°á·Ð: °í·ÉÀÇ È¯ÀÚ ¹× üºÎÀÇ Àå»óÇÇ È»ý Á¤µµ°¡ ½ÉÇÒ¼ö·Ï À§ ¼±Á¾À̳ª Á¶±â À§¾Ï ȯÀÚ¿¡¼ µ¿½Ã ´Ù¹ß¼º º´º¯À» °®´Â ºñÀ²ÀÌ ³ô¾Æ, ÀÌ·¯ÇÑ È¯Àڵ鿡°Ô¼´Â µ¿½Ã¼º º´º¯ ¿©ºÎ¿¡ ´ëÇÑ ¼¼½ÉÇÑ ÁÖÀÇ°¡ ÇÊ¿äÇÏ´Ù
Background/Aims: This study was performed to evaluate the clinical and histological differences of background gastric mucosa between single lesion and multiple synchronous lesions in gastric adenoma or early gastric cancer.
Methods: A total of 682 patients with gastric adenoma or early gastric cancer were prospectively enrolled after endoscopic submucosal dissection (ESD) between April 2005 and November 2007. Rapid urease test and biopsies in surrounding normal mucosa of gastric antrum and body were performed to evaluate H. pylori infection and basal histology.
Results: A total of 545 patients with single lesion and 137 patients with multiple synchronous lesions were enrolled. Age of patients with multiple synchronous lesions (64¡¾8 years) was significantly older than patients with single lesion (61¡¾9 years) (p=0.037). In multiple regression analysis, old age and severe intestinal metaplasia of the body was significantly related with multiple synchronous lesions in gastric tumor. However, sex, H. pylori infection and histopathology of adenoma or adenocarcinoma had no significant relationship with multiplicity of lesion.
Conclusions: Multiple synchronous lesions were significantly related with old age and intestinal metaplasia of the body in gastric tumor. (The Korean Journal of Helicobacter and Upper Gastrointestinal Research 2008;8:26-31)
