Abstract

새롭게 개편된 DSM-5의 등장으로 아스퍼거 증후군은 자폐 스펙트럼 장애로 흡수되어 사라지게 되었다. 이를 둘러싼 그동안의 논쟁과 연구동향을 알아보기 위하여 이 연구에서는 총 19편의 국외논문이 분석되었다. 그 결과 아스퍼거 증후군과 관련한 DSM-5의 논쟁은 1) 새로운 진단명 ASD(autism spectrum disorders)와 하위유형 논쟁, 2) DSM-5의 민감도와 특이도 논쟁, 3) 장애 이름붙이기(labeling)와 지원딜레마 등으로 정리되었다. 이에 대한 지금까지의 연구결과는 단일 진단명의 출현에 대한 긍정적인 측면과 하위유형 연구의 축소에 대한 우려로 정리되었으며, 일부 아스퍼거 증후군 집단의 경우 DSM-5 진단기준으로 재진단시 ASD가 아닌 사회적 의사소통 장애(social communication disorder)로 진단될 수 있다는 점 등이 지적되었다. 또한 독특한 아스퍼거 증후군 집단의 정체성 및 추후 지원의 연속성 문제에 대한 전망과 고려사항이 함께 논의되었다.

Asperger syndrome (AS) will be merged into autism spectrum disorders under the DSM-5 criteria. In the study, a total of 19 studies has been selected and analyzed in order to review and update the current issues and trends related to this dramatic change. As a result, the following areas were discussed in terms of AS and the DSM-5 debates: (1) the emergence of a new diagnosis, ASD and vanishing subcategories, (2) sensitivity and specificity of DSM-5 diagnostic criteria, (3) the dilemma of labeling. The results include pros and cons of creating a single diagnostic term. In addition, concerns that the new criteria for ASD may reduce the number of diagnoses significantly and the confusing concept of the new diagnosis, social communication disorder were also included. Finally, unique identity of ‘Aspie culture’ of people with AS as well as further considerations about continuity of support for the population and their families were discussed.
