Abstract

¸ñÀû: ´ëºÎºÐÀÇ ÂêÂê°¡¹«½Ãº´ ȯÀÚ´Â Á¤»ó ½ÉÀüµµ¸¦ º¸À̳ª ÀϺο¡¼­ ºñƯÀÌÀû ½ÉÀüµµ º¯È­ ¹× ½É°¢ÇÑ ½ÉÀüµµ º¯È­¿¡ ´ëÇÑ Áõ·Ê°¡ º¸°íµÈ ¹Ù ÀÖ´Ù. ÇÏÁö¸¸ ÀÌ¿¡ ´ëÇÑ ¿¬±¸´Â ¾ÆÁ÷ ºÎÁ·ÇÑ ½ÇÁ¤À¸·Î º» ¿¬±¸¿¡¼­´Â ÂêÂê°¡¹«½Ãº´ ȯÀÚÀÇ ½ÉÀüµµ º¯È­¿¡ ´ëÇÏ¿© ¾Ë¾Æº¸°íÀÚ ÇÏ¿´´Ù.

¹æ¹ý: 2008³âºÎÅÍ 2012³â±îÁö ´ëÇк´¿ø¿¡ ÂêÂê°¡¹«½Ãº´À¸·Î ÀÔ¿øÇÑ È¯ÀÚ¸¦ ´ë»óÀ¸·Î ÈÄÇâÀû ¿¬±¸¸¦ ½ÃÇàÇÏ¿´´Ù. ±âÀú ½ÉÁúȯÀÚ¸¦ Á¦¿ÜÇÑ 106¸íÀÇ ÂêÂê°¡¹«½Ã ȯÀÚ¸¦ ´ë»óÀ¸·Î ³»¿ø ´ç½Ã ½ÉÀüµµ, ÃßÀû ½ÉÀüµµ ¼Ò°ß, Àα¸ÇÐÀû Ư¼º ¹× °Ë»ç½Ç ¼Ò°ßÀ» Á¶»çÇÏ¿´´Ù.

°á°ú: Àüü ȯÀÚ Áß 30% ÀÌ»óÀÇ È¯ÀÚ°¡ ³»¿ø ´ç½Ã ºñÁ¤»ó ½ÉÀüµµ ¼Ò°ßÀ» º¸¿´´Ù. ³»¿ø ´ç½Ã ½ÉÀüµµ°¡ ºÎÁ¤¸ÆÀ» º¸¿´´ø ±º¿¡¼­ Ç÷¾× ¿ä¼Ò Áú¼Ò ¼öÄ¡ ¹× ¿£Æ¼ ÇÁ·Î-ºñ¿£ÇÇ ¼öÄ¡°¡ ³ô¾ÒÀ½À» ¾Ë ¼ö ÀÖ¾ú´Ù. ±×¸®°í ÃßÀû ½ÉÀüµµ °Ë»ç¿¡¼­ ºñÁ¤»ó ¼Ò°ßÀ» º¸ÀÌ´Â ±ºÀÇ Ç÷¾× ¿ä¼Ò Áú¼Ò ¹× Å©·¹¾ÆÆ¼´Ñ ¼öÄ¡°¡ ³ô¾Ò´Ù. ¶ÇÇÑ ºñÁ¤»ó ½ÉÀüµµ°¡ º¯È­ ¾øÀÌ Áö¼ÓµÈ ±ºÀº ÁßȯÀÚ½Ç ÀÔ½Ç ºñÀ²ÀÌ ³ô°í ÀÔ¿ø ±â°£ÀÌ ´õ ±æ´Ù´Â »ç½ÇÀ» È®ÀÎÇÏ¿´´Ù.

°á·Ð: ÂêÂê°¡¹«½Ãº´ ȯÀÚ¿¡¼­ ½ÉÀüµµ º¯È­°¡ ÁßÁõµµ¿Í ¿¬°üµÇ¾î ÀÖÀ» ¼ö ÀÖÀ¸³ª ´ÜÀÏ ±â°ü ¿¬±¸·Î ÇâÈÄ Ãß°¡ÀûÀÎ ´ë±Ô¸ð ¿¬±¸°¡ ÇÊ¿äÇÒ °ÍÀ¸·Î »ý°¢µÈ´Ù.
Methods: The ECG and demographic data of patients infected with Orientia tsutsugamushi seen at Dongsan Medical Center from January 2008 to June 2012 were reviewed retrospectively. Patients with pre-existing cardiac disease were excluded. The initial and follow-up ECG findings, demographic characteristics, and laboratory findings was examined.

Results: The group in which the initial ECG showed an arrhythmia had a higher blood urea nitrogen (BUN) level than the normal and ischemic groups (median 26.0 vs. 15.0 vs. 15.5 mg/dL, p = 0.041). The group with a sustained abnormality in the follow-up ECG had higher BUN (median 17.0 vs. 15.0 mg/dL, p = 0.022) and creatinine (median 1.30 vs. 0.90 mg/dL, p = 0.026) levels than the normal ECG group. In addition, the group with a sustained abnormality in the follow-up ECG had a higher rate of intensive care unit (ICU) admission (30.8% vs. 5.3%, p = 0.015) and longer treatment period (average 9 vs. 6 days, p = 0.027) than the other groups.

Conclusion:
In our study, more than 30% of the patients with scrub typhus showed ECG abnormalities. Those with an arrhythmic ECG change on presentation had high BUN levels. The sustained ECG abnormality group had high BUN and creatinine levels, and an increased ICU admission rate and length of stay. We should pay attention to the ECG findings on presentation and during the treatment of scrub typhus.
