Abstract

본 연구는 보건복지부가 2007년부터 실시하고 있는 맞춤형 방문건강관리 사업의 일환으로 진행되고 있는 일 보건소의 고혈압 관리 프로그램에 등록된 고혈압 환자의 혈압, 혈중지질 측정치와 고혈압 관련 지식 및 일상생활 실천에 미치는 효과를 파악하기 위하여 설계된 종단적 연구로서, 비동등성 대조군 유사 실험 설계이다. 구체적인 연구 목표는 첫째, 고혈압 사례관리군의 시기별 혈압, 혈중 지질 측정치의 변화, 고혈압에 관한 지식, 건강생활 실천 이행 변화를 파악하고, 둘째, 고혈압 사례관리군과 일반관리군의 혈압, 혈중 지질 측정치, 고혈압에 관한 지식, 건강생활 실천 이행 등의 변화를 비교 분석함으로서 고혈압사례관리 프로그램의 효과를 규명하고자 함이다. 연구결과를 통하여 고혈압 사례관리를 받는 환자들이 일반관리를 받는 고혈압 환자들보다 수축기혈압과 이완기 혈압 조절효과가 우수한 것으로 나타났으며, 중성지방, 총 콜레스테롤, 고밀도 지방단백질, 저밀도 지방단백질의 개선은 뚜렷하게 나타나지 않았으며. 고혈압 관련 지식, 건강생활 실천정도는 8주 후에 증가되었다가 추후측정에서는 감소됨을 알 수 있었다

Purpose: The purpose of this study was to investigate the effects of case management service for hypertensive elderly patients through the customized visiting home health care program.

Methods: Non-equivalent control group pretest-posttest design was employed. Data were collected from April 2007 to January 2009. Among a total of fifty subjects, 25 subjects were belonged to the case management group and the others were belonged to the general management group.

Results: 1) In experimental group, significant increasing rate of number of people within normal blood pressure by time, but no significant increasing rate of number of people within normal blood neutral fat, total cholesterol, HDL, & LDL. 2) In experimental group, the score of knowledge about disease and healthy life practice were increased on 8 week period but decreased on follow up period. Number of people within normal blood HDL and mean score of healthy life practice were significantly higher than control group on follow up period.

Conclusion: Case management service was effective on controling blood pressure but partly effective on blood lipid, knowledge about disease, and healthy life practice in hypertensive patients. Further study is needed to conduct a periodic re-evaluation of the effect of case management and to verify an optimal interval to provide the case management service.
