Abstract

We report a case of extensive compartment syndrome of the bilateral upper and lower extremities associated with peripheral sensorimotor neuropathy resulting from skeletal muscle swelling secondary to influenza A infection. A 42-year-old man presented to the emergency room with all four limbs pain, paresis, muscle swelling and renal failure. Rapid influenza antigen detection result was positive. Serum creatine phosphokinase and myoglobin were elevated and T2 weighted MRI demonstrates muscle swelling, rim enhancement around high signal intensity area in both forearms and calves. He was diagnosed with acute viral myositis going to rhadomyolysis combined with compartment syndrome. Fasciotomy could not be done. At 1 monthfollow- up, weakness in distal limbs remained and electrodiagnostic study revealed severe axonal neuropathy of the bilateral upper and lower extremities motor and sensory nerves.
