Abstract
Áö¼ÓÀû ½Å ´ëü ¿ä¹ý(CRRT)Àº ±Þ¼º ½Å¼Õ»óÀÌ ÀÖ´Â ÁßÁõ ¼Ò¾ÆÀÇ Ä¡·á·Î Á¡Â÷ »ç¿ëÀÌ ´Ã¾î³ª°í ÀÖÀ¸¸ç CRRTÀÇ ±â¼ú°ú ½ÇÁ¦ »ç¿ë¹ýÀÌ ¹ß´ÞÇÏ¸é¼ ÀÛÀº ¿µ¾Æ³ª ½Å»ý¾Æ¿¡¼µµ ±× »ç¿ëÀÌ Á¶±Ý¾¿ ´Ã¾î³ª°í ÀÖ´Ù. °í¾Ï¸ð´Ï¾ÆÇ÷ÁõÀ̳ª ü¿Ü¸·»ê¼ÒÈ ÀåÄ¡(ECMO) Ä¡·á Áß¿¡ ¹ß»ýÇÑ ±Þ¼º ½Å¼Õ»ó µîÀÇ °æ¿ì CRRT°¡ ¾ÈÀüÇϰí È¿°úÀûÀÎ Ä¡·á°¡ µÉ ¼ö ÀÖÀ¸³ª, Ç÷°ü Á¢±ÙÀ̳ª ÃâÇ÷ ±×¸®°í ½Å»ý¾Æ Àü¿ë CRRT deviceÀÇ ºÎÀç·Î ÀÎÇÑ ¿©·¯ °¡Áö Á¦ÇÑÁ¡ÀÌ ÀÖ´Ù. ÀÌ Á¾¼³¿¡¼´Â ±âº»ÀûÀÎ CRRTÀÇ ¿ø¸®¸¦ ¾Ë¾Æº¸°í ½Å»ý¾Æ¿Í ¿µ¾Æ¿¡¼ Ưº°È÷ °í·ÁÇØ¾ß ÇÒ »çÇ׵鿡 ´ëÇØ ¾Ë¾Æº¸°íÀÚ ÇÑ´Ù.
Continuous renal replacement therapy (CRRT) has become the preferred dialysismodality to support critically ill children with acute kidney injury. As CRRT technologyand clinical practice advances, experiences using CRRT on small infants andneonates have increased. In neonates with hyperammonemia or acute kidneyinjury during extracorporeal membrane oxygenation (ECMO) therapy, CRRT canbe a safe and effective technique. However, there are many limitations of CRRT inneonates, including vascular access, bleeding complications, and lack of neonatespecificdevices. This review discusses the basic principles of CRRT and the specialconsiderations when using this technique in neonates and infants.
