Abstract
¸ñÀû: ¿øÀ§ ¿ä°ñ °ñÀýÀÇ ¼öÀåÃø Àá±è ±Ý¼ÓÆÇ °íÁ¤¿¡¼ ¿øÀ§ ³ª»ç¸øÀÇ °³¼ö¿¡ µû¸¥ ¹æ»ç¼±ÇÐÀû °á°úÀÇ Â÷À̸¦ ¾Ë¾Æº¸°íÀÚ ÇÏ¿´´Ù.
¹æ¹ý: 2011³âºÎÅÍ 2012³â±îÁö ¼öÀåÃø Àá±è ±Ý¼ÓÆÇÀ» ÀÌ¿ëÇÏ¿© ¿øÀ§ ¿ä°ñ °ñÀýÀ» Ä¡·áÇϰí 3°³¿ù ÀÌ»ó Ã߽ð¡ °¡´ÉÇÏ¿´´ø 176¸íÀÇ È¯ÀÚ(³²ÀÚ 36¸í, ¿©ÀÚ 140¸í, Æò±Õ¿¬·É 60¼¼)¸¦ ´ë»óÀ¸·Î ÇÏ¿´´Ù. ¿øÀ§ºÎ Àá±è ³ª»ç¸øÀÇ °³¼ö´Â ¼úÀÚÀÇ ¼±È£µµ ¹× ±â±¸ÀÇ Á¾·ù¿¡ µû¶ó °áÁ¤ÇÏ¿´´Ù. ¼ö¼ú Á÷ÈÄ ¹× ÃÖÁ¾ Ã߽à ½Ã ¿ä°ñ ±æÀÌ, ¿ä°ñ °æ»ç, ¼öÀå °æ»ç, ô°ñ º¯À§¸¦ ÃøÁ¤ÇÏ¿´°í, ±× °á°ú¸¦ ¿øÀ§ ³ª»ç¸øÀÇ °³¼ö°¡ 5°³ ÀÌÇÏÀÎ ±×·ì°ú 6°³ ÀÌ»óÀÎ ±×·ìÀ¸·Î ³ª´©¾î ºñ±³ÇÏ¿´´Ù.
°á°ú: ¸ðµç ¹æ»ç¼±ÇÐÀû ÀÎÀÚ¿¡¼ ¿øÀ§ ¿ä°ñÀÇ ÀüÀ§ Á¤µµ´Â ±×·ì °£¿¡ Â÷À̰¡ ¾ø¾ú´Ù. ¿øÀ§ ³ª»ç¸øÀÇ °³¼ö°¡ 5°³ ÀÌÇÏÀÎ ±×·ìÀÇ Ã´°ñ º¯À§´Â Æò±Õ 0.6 mm, 6°³ ÀÌ»óÀÎ ±×·ìÀº Æò±Õ 0.4 mm·Î ¾ç ±º °£¿¡ Â÷À̸¦ ¹ß°ßÇÒ ¼ö ¾ø¾ú´Ù(p=0.772). °ñÀýÀÇ ÀüÀ§ Á¤µµ´Â °ñÀýÀÇ Çüųª ¿øÀ§ ³ª»ç¸øÀÇ °³¼ö¿Í »ó°ü°ü°è°¡ ¾ø¾úÀ¸¸ç, °ñÀýÀÇ °íÁ¤ ½ÇÆÐ°¡ ¹ß»ýÇÑ °æ¿ìµµ ¾ø¾ú´Ù.
°á·Ð: ¿øÀ§ ¿ä°ñ °ñÀýÀÇ ¼öÀåºÎ Àá±è ±Ý¼ÓÆÇ °íÁ¤¼ú¿¡¼ ¿øÀ§ Àá±è ³ª»ç¸øÀº 5°³ ÀÌÇϷεµ ÃæºÐÇÑ °íÁ¤·ÂÀ» ¾òÀ» ¼ö ÀÖ
À¸¸ç, 6°³ ÀÌ»óÀÇ ¿øÀ§ ³ª»ç¸ø »ðÀÔÀº Ưº°ÇÑ °æ¿ì¸¦ Á¦¿ÜÇϰí´Â ºÒÇÊ¿äÇÒ °ÍÀ¸·Î ÆÇ´ÜµÈ´Ù.
Purpose: The purpose of this study was to determine whether the number of distal locking screws affected the final radiologic results after volar plate fixation for distal radius fractures.
Methods: We retrospectively identified 176 patients (male, 36; female, 140; average, 60 years) who had distal radius fractures treated with open reduction and volar plate fixation between 2011 and 2012. The number of screws used for distal fixation was determined according to the surgeon¡¯s preference and the type of plate used. Radiologic parameters and their displacements were measured postoperatively and at final follow-up. The results of using 4 or 5 distal locking screws were compared with those of using more than 6 distal locking screws.
Results: There was no significant displacement in fracture fragment when using 4 or 5 distal locking screws compared with using more than 6 distal locking screws. Mean displacement in ulnar variance was 0.6 mm in group with less than 5 screws, and the displacement was 0.4 mm in group with more than 6 screws (p=0.772). Secondary displacement was not correlated with fracture type or the number of distal locking screws. There was no fixation failure during the study period.
Conclusion: It seems that 4 or 5 distal locking screws are strong enough to prevent a significant loss of fracture reduction. Filling every distal screw hole is not recommended to limit cost and avoid extensor tendon complications.
