Abstract

목적: 저자들은 수부 재건 수술에 새로운 전층 피부이식 공여부로 귀뒤쪽 피부를 사용하였으며 그 결과를 보고하고자 한다.

방법: 2006년 1월부터 2013년 3월까지 수부에 피부이식이 필요한 55명의 환자를 대상으로 귀뒤쪽 피부를 이용하여 전층 피부이식술을 실시하였다. 1년 이상 경과 관찰 후 피부이식을 한 부위와 주변 피부의 색감의 차이 및 공여부의 흉터를 환자 본인과 의사 3명이 육안적으로 확인하고 5점을 만점으로 하여 만족도를 조사하였다.

결과: 화상 후 형성된 흉터 반흔으로 피부이식을 받았던 1예에서 부분적인 괴사가 있었으나 보존적 치료로 치유되었으며 나머지 54예에서 이식된 피부는 문제없이 생착되었다. 피부이식부에 대한 환자 개인의 주관적 만족도는 4.07, 의사 세 명의 평가에 의한 객관적 만족도는 4.18로 결과가 좋았으며, 공여부 반흔에 대한 평가에서도 환자 개인의 주관적 만족도 4.93, 의사 세 명의 평가에 의한 객관적 만족도 4.98로 결과가 매우 좋았다.

결론: 수부 수술에서 후이개부위를 사용한 전층 피부이식술은 주변 피부와 조화가 좋으며, 공여부 흉터 또한 눈에 잘 띄지 않아 미용적으로 우수한 결과를 얻을 수 있는 방법이다.

Purpose: We used the retroauricular area skin as an alternative full-thickness skin donor site in the hand reconstruction surgery.

Methods: From January 2006 to March 2013, 55 patients observed more than 1 year, were recruited for the study. In case of requiring the skin graft on hand, we grafted skins were harvested from the retroauricular area on hand. To assess the subjective and objective satisfaction, the patients themselves and three doctors compared the skin graft area’s color with adjacent skin and estimated the donor site scar on a five-point scale.

Results: A partial necrosis was seen in one case who received skin graft because of a burn scar contracture, but it was cured with conservative treatment. In other cases, the skin graft was well taken without any specific problems. The retroauricular skin graft showed good color match with adjacent skin and less pigmentation. Donor site scar was not noticeable. As a result, the patient’s subjective satisfaction (4.07) and doctor’s objective satisfaction (4.18) about skin graft were very good. And the patient’s subjective satisfaction (4.93) and doctor’s objective satisfaction (4.98) about donor site scar were also very good.

Conclusion: We obtained the favorable result and the patients satisfaction by using the retroauricular area as a full thickness skin graft donor site in hand.
