Abstract

목적 : 본 연구는 경도 인지장애 선별을 위한 전산화 인지기능 평가의 체계적 고찰을 통해 임상 및 연구의 기초 자료를 제공하고자 하였다.

연구방법 : 2015년 11월 한 달 동안 Embase, PubMed, PsycINFO, The Cochrane Library에서“computerized”OR“computer-based”AND“cognitive assessment”OR“screening test”AND“mild cognitive impairment”의 키워드를 검색하여 최종적으로 12개의 연구에서 9개의 전산화 인지기능 평가를 추출하였다. 연구의 질적 수준, 평가 구성 비율, 대상자 선정기준, 평가 운영체제, 시행방법, 평가영역, 신뢰도, 민감도 및 특이도를 분석하였다.

결과 : 12편의 연구 질적 수준을 분석한 결과, 수준 Ⅱ의 연구는 11편으로 전체의 91.7%였고 수준 Ⅲ 단계의 연구는 1편으로 전체의 8.3%를 차지하여 전반적인 근거수준은 대체로 높았다. 경도 인지장애 대상자 선정을 위해서 간이정신건강상태검사(Mini-Mental State Examination; MMSE)을 포함해 다양한 평가를 사용하고 있었다. 9개의 전산화 인지기능 평가 중 알츠하이머형 치매 선별도 가능한 것은 3개였다. 대부분의 평가는 컴퓨터 기반으로 3~30분 이내의 시간에 시행되고 있었으며, 키보드 또는 마우스를 이용한 자기시행법이 가장 많았고 터치를 이용한 방법이 다음으로 많았다. 각 평가별로 평가영역에 차이가 있었으나 기억력 평가는 모든 평가에 포함되어 있었으며, 평가의 신뢰도는 대체로 높았다. 평가의 민감도 및 특이도는 지필평가와 유사하거나 낮은 것으로 나타났다.

결론 : 본 연구를 통하여 경도 인지장애 선별을 위한 전산화 인지기능 평가의 특징을 종합적으로 알아보았다. 연구결과는 추후 경도 인지장애 선별을 위한 전산화 인지기능 평가 개발을 위한 기초자료로 활용될 수 있을 것이다.

Objective: This study was conducted to systematically review computerized cognitive function tests for the screening of mild cognitive impairment (MCI).

Methods: We searched papers published in Embase, PubMed, PsycINFO, and the Cochrane Library. The key words used in the search were "computerized, "computer-based" and cognitive assessment", "screening test" and "MCI" In totla of 12 papers, nine computerized cognitive function tests were extracted.

Results: The evidence of the 12 papers ranged from level Ⅱ (91.7%) to level Ⅲ(8.3%), indicating that the overall evidence level was high. The total number of computerized cognitive function tests that can be used to screen Alzheimer's type dementia in addition to MCI was three. The subjects were selected using a variety of tests including a Mini-Mental State Examination. Most of the personal computer based tests were conducted within 3 to 30 minutes using a keyboard or mouse through a self administration. Although the test areas differ between the systems used, the memory area was included in all tests. The reliability of the tests was high and the sensitivity and specificity of the tests showed were shown to be similar to or lower than paper and pencil based tests.

Conclusions: These results can be used as the basic data for developing a computerized cognitive function test for the screening of MCI.
