Abstract

자궁의 다형성 횡문근육종은 폐경 후 여성에서 매우 드물게 발생하는 예후가 나쁜 악성종양이다. 65세 여성이 질 출혈과 하복부 통증을 호소하였다. 질 속에 커다란 괴사성 종괴가 빠져나와 있었고, 자궁 체부의 암육종이 의심되었다. 자기공명영상 검사에서 이질성 신호를 보이는 거대종괴가 자궁 안에 발견되었다. 난소난관절제술과 골반임파절절제술을 포함한 근치적 자궁절제술을 시행하였다. 병리결과 자궁체부의 순수 다형성 횡문근육종으로 최종 확인되어 ifosfamide와 cisplatin을 이용한 보조화학요법을 3차례 시행하였다. 치료 후 추적 관찰하던 중 별 증상이 없던 환자가 수술 후 10개월째에 갑작스런 흉통을 호소하였다. 양전자방출단층촬영(PET/CT) 검사를 하니 흉부와 엉덩뼈에 커다란 전이병변이 발견되었다. 이후 여러 차례의 방사선치료에도 불구하고 위장 전이로 인한 심한 토혈로 사망하였다. 본 증례는 한국에서는 최초로 보고되는 자궁의 다형성 횡문근육종으로서 이 종양 재발의 조기진단에 PET/CT가 유용할 것이라고 제안하는 바이다.

Pleomorphic rhabdomyosarcoma of the uterus is an extremely rare malignant neoplasm with a poor prognosis. A 65-year-old woman presented with vaginal bleeding and suprapubic abdominal pain. A large necrotic mass prolapsed into the vaginal canal. Magnetic resonance imaging demonstrated a huge mass with heterogeneous signal intensity in the uterus. Radical hysterectomy including bilateral salpingo-oophorectomy and pelvic lymph node dissection was performed. Pathology revealed pure pleomorphic rhabdomyosarcoma of the uterine corpus, and the patient received 3 cycles of adjuvant chemotherapy using Ifosfamide and cisplatin. The patient was healthy until a sudden chest pain ten months later. PET-CT demonstrated tumor recurrence in her thorax and iliac bone. She died of massive hematemesis due to metastasis to the stomach. We present the first reported case of pleomorphic rhabdomyosarcoma of the uterus from South Korea, and suggest that PET-CT can be useful in the early detection of tumor recurrence.
