Abstract

¸ñ Àû: º» ¿¬ÀÇ ¸ñÀûÀº Æó¼â¼º¼ö¸é¹«È£ÈíÁõÀ¸·Î ÇÏ¾Ç Àü¹æÀ̵¿ÀåÄ¡ Ä¡·á¸¦ ½ÃÇàÇÑ È¯Àڵ鿡°Ô ÇϾÇÀü¹æÀ̵¿ÀåÄ¡ÀÇ À¯¿ë¼ºÀ» È®ÀÎÇÏ°í, ÇϾÇÀü¹æÀ̵¿Ã¢Ä¡ Ä¡·á½Ã ÇϾÇÀÇ Àü¹æÀ̵¿·®ÀÌ ¹ÌÄ¡´Â ¿µÇâÀ» Æò°¡ÇÏ°íÀÚ ÇÏ¿´´Ù.

¹æ ¹ý: 2007³â 1¿ùºÎÅÍ 2009³â 2¿ù±îÁö ÄÚ°ñÀÌ ¹× ¼ö¸é¹«È£ÈíÀ» ÁÖ¼Ò·Î ºÐ´ç¼­¿ï´ëÇб³º´¿ø¿¡ ³»¿øÇÏ¿© ¾ß°£¼ö¸é´Ù¿ø°Ë»ç¸¦ ÅëÇØ Æó¼â¼º¼ö¸é¹«È£ÈíÁõÀ¸·Î Áø´Ü¹ÞÀº ȯÀÚ Áß¿¡¼­, Ä¡·á¹ýÀ¸·Î ÇϾÇÀü¹æÀ̵¿ÀåÄ¡¸¦ ¼±ÅÃÇÑ ÃÑ 86¸íÀÇ È¯ÀÚ¸¦ ÈÄÇâÀûÀ¸·Î Æò°¡ÇÏ¿´´Ù. °á °ú: ÃÑ 86¸íÀÇ È¯ÀÚ Áß 65¸í(75.6%)¿¡°Ô¼­ ¼º°øÀûÀÎ °á°ú¸¦ ¾ò¾ú´Ù. °æ°ú°üÂû ±â°£ Áß¿¡ 28¸íÀÇ È¯ÀÚ¿¡°Ô¼­ °æ¹ÌÇÑ ÀüÄ¡ºÎ¿Í ÅΰüÀýÀÇ ºÒÆí°¨ÀÌ º¸°íµÇ¾úÀ¸¸ç, ƯÈ÷ ÇϾÇÀÇ Àü¹æÀ̵¿·®¿¡ µû¸¥ ºÒÆí°¨Àº 7.0 mm ÀÌ»óÀÇ È¯ÀÚ¿¡°Ô¼­ µÎµå·¯Áö°Ô º¸°íµÇ¾ú´Ù. ÇϾÇÀÇ Àü¹æÀ̵¿·®°ú ÀÓ»ó °á°ú¿Í´Â Á÷Á¢ÀûÀÎ °ü·ÃÀº ¾ø¾ú´Ù.

°á ·Ð: ÁßÁõµµ¿Í »ó°ü¾øÀÌ ÇϾÇÀü¹æÀ̵¿ÀåÄ¡´Â Æó¼â¼º ¼ö¸é¹«È£ÈíÁõ ȯÀÚÀÇ È¿°úÀûÀÎ Ä¡·á¹æ¹ýÀÇ Çϳª·Î ¼±ÅÃµÉ ¼ö ÀÖ´Ù. ÀáÀçÀûÀÎ Ä¡°úÀûÀÎ ºÎÀÛ¿ëÀ» ¹æÁöÇϱâ À§ÇÏ¿© ÇϾÇÀü¹æÀ̵¿ÀåÄ¡ÀÇ Ä¡·á½Ã¿¡ ÇϾÇÀÇ Àü¹æÀ̵¿·®À» ¿ì¼±ÀûÀ¸·Î °í·ÁÇÏ¿©¾ß ÇÒ °ÍÀÌ´Ù.
Objectives: The purposes of this study were to estimate the effect of mandibular advancement device (MAD) and to evaluate the influence of the advancement amount of mandible in the application of MAD for obstructive sleep apnea (OSA) patients.

Methods: From the patients who were diagnosed as OSA by polysomnographic study at Seoul National University Bundang Hospital from January 2007 to February 2009, the patients who chose MAD as treatment option were included in this study. All the patients¡¯ data including clinical records and polysomnographic studies (both pre- and post-treatment) were reviewed and analyzed.

Results: Successful results were obtained in 65 patients of 86 patients (75.6%). In the follow-up period, mild discomfort of anterior teeth or temporomandibular joint (TMJ) were described in 28 patients, especially in the cases the amount of mandibular advancement were more than 7.0 mm. There was no direct relationship between the amount of mandibular advancement and clinical outcome.

Conclusion: MAD was effective treatment option for the OSA patients regardless of severity. For the prevention of potential dental complications, the amount of mandibular advancement should be considered at the time of MAD treatment.
