Abstract
¸ñ Àû: Neer classification type IIÀÇ ¼â°ñ ¿øÀ§ºÎ °ñÀý¿¡¼ K-°¼± ¹× ±äÀå´ë °¼± °íÁ¤¼ú ÈÄ ¹æ»ç¼±ÇÐÀû ¹× ÀÓ»óÀû °á°ú¿¡ ´ëÇØ ºÐ¼®ÇÏ°íÀÚ ÇÏ¿´´Ù.
´ë»ó ¹× ¹æ¹ý: 2002³â 3¿ùºÎÅÍ 2011³â 5¿ù±îÁö Neer Á¦IIÇü ¿øÀ§ ¼â°ñ °ñÀý·Î K-°¼± °üÇ÷Àû Á¤º¹ÇÏ¿¡ Àå·Â´ë °¼± °íÁ¤À» ÀÌ¿ëÇÏ¿© ¼ö¼úÀû Ä¡·á¸¦ ¹ÞÀº ÈÄ 1³â ÀÌ»ó Ã߽ð¡ °¡´ÉÇÏ¿´´ø 26¿¹(IIa 15¿¹, IIb 11¿¹)¸¦ ´ë»óÀ¸·Î ÇÏ¿´´Ù. °á °ú: ¹æ»ç¼±ÇÐÀû °ñÀ¯ÇÕÀº Æò±Õ 11.7ÁÖ(8-20ÁÖ)¿¡ °üÂûµÇ¾ú´Ù. ¼ö¼ú ÈÄ ½Ã°¢ ÅëÁõ Á¡¼ö´Â Àüü ¿¹¿¡¼ ¼ö¼ú ÈÄ Æò±Õ 1.23¡¾2.75 (p£¼0.05)·Î °¨¼ÒÇÏ¿´´Ù. UCLA score´Â ¼ö¼ú ÈÄ Æò±Õ 33.5¡¾2.15 (p£¼0.05)·Î È£ÀüµÇ¾ú´Ù.
°á ·Ð: Á¦IIÇü ¿øÀ§ ¼â°ñ °ñÀýÀÇ ¿©·¯ Ä¡·á ¹æ¹ý Áß K-°¼± ¹× ±äÀå´ë °íÁ¤¼úÀ» ½ÃÇàÇÏ¿© ¹æ»ç¼±ÇÐÀû, ÀÓ»óÀûÀ¸·Î ºñ±³Àû ¸¸Á·ÇÒ ¸¸ÇÑ °á°ú¸¦ º¸¿´´Ù. ÀÌ ¼ú½ÄÀº ¼ö±â°¡ ºñ±³Àû ½±°í ÇÕº´ÁõÀÌ ÀûÀ¸¸ç Á¶±â¿¡ °ß°üÀý ¿îµ¿ÀÌ °¡´ÉÇÏ¿© ¼ú ÈÄ ÀÓ»óÀû °á°ú°¡ ¿ì¼öÇÑ ¼ö¼ú ¹æ¹ýÀ̶ó°í »ý°¢µÈ´Ù.
Purpose: The purpose of this study is to evaluate the radiologic and clinical outcomes after tension band wire fixation of Neer type II distal clavicle fractures.
Materials and Methods: Twenty-six patients with Neer type II distal clavicle fractures who underwent tension band wire fixation from March 2002 to May 2011 were included in the study. Fifteen cases were classified as Neer type IIa and 11 cases as type IIb. The postoperative mean follow-up period was 14.3 months. Clinical and radiologic evaluation was performed at two weeks, six weeks, three months, six months, and 12 months postoperatively.
Results: Bony union on X-rays was observed at an average of 11.7 weeks (range 8-20 weeks) postoperatively. The overall visual analogue scale score for pain was 1.23¡¾2.75 postoperatively. The overall postoperative University of California at Los Angeles score increased to 33.5¡¾2.15 from the preoperative score of 21.6¡¾1.91 (p£¼0.05).
Conclusion: Among various methods of treatment for Neer type II distal clavicle fracture, K-wire and tension band fixation was used and relatively satisfactory radiological and clinical results were obtained. This surgical method yields excellent clinical results, owing to its relatively easy technique, fewer complications, and allowance of early rehabilitation.
