Abstract
¸ñÀû: °ñ¹Ýȯ ¼Õ»ó¿¡¼ ¹ß»ýÇÑ ½Å°æÀû ¼Õ»ó Á¤µµ¸¦ ÆÄ¾ÇÇÏ°í ½Å°æ ¼Õ»ó¿¡ ¿µÇâÀ» ÁÖ´Â À§Çè ÀÎÀÚ¸¦ ¾Ë¾Æº¸°íÀÚ ÇÑ´Ù.
´ë»ó ¹× ¹æ¹ý: 2010³â 3¿ùºÎÅÍ 2013³â 5¿ù±îÁö °ñ¹Ýȯ ¼Õ»ó 62¿¹¸¦ ´ë»óÀ¸·Î ÇÏ¿´À¸¸ç ½Å°æÀû ¼Õ»óÀÌ ÀǽɵǴ °æ¿ì ±ÙÀüµµ °Ë»ç¸¦ ½ÃÇàÇÏ¿© È®ÀÎÇÏ¿´´Ù. µ¿¹Ý ¼Õ»ó, °ñÀý ºÐ·ù, ÈĹæÈ¯ ÀüÀ§ Á¤µµ¿¡ ´ëÇÑ Á¶»ç¸¦ ÇÏ¿© ½Å°æÀû ¼Õ»ó¿¡ ¿µÇâÀ» ¹ÌÄ¡´Â ÀÎÀÚ¸¦ ºÐ¼®ÇÏ¿´´Ù.
°á°ú: Àüü 62¿¹ Áß AO/OTA ºÐ·ù»ó AÇüÀÌ 7¿¹, BÇüÀÌ 37¿¹, CÇüÀÌ 18¿¹¿´´Ù. ôÃß °ñÀýÀÌ µ¿¹ÝµÈ °æ¿ì°¡ 25¿¹(40%)¿´°í, °ñÀýÀÇ ÀüÀ§ Á¤µµ´Â Æò±Õ 7 mm (1-50 mm)¿´´Ù. ½Å°æÀû ¼Õ»óÀÌ ÀÖ´Â °æ¿ì´Â 20¿¹¿´À¸¸ç ÀÌ Áß °ñ¹Ýȯ ¼Õ»ó°ú °ü·ÃµÈ ½Å°æÀû ÀÌ»óÀº 13¿¹(21%)¿´´Ù. ¿äõ°ñ ½Å°æÃÑ º´º¯ 5¿¹, Á¦5¿äÃß ¹× Á¦1õÃß ½Å°æ ±Ùº´Áõ 5¿¹, Æó¼â ½Å°æ ¼Õ»ó 2¿¹, ¿ÜÃø ´ëÅð ÇǺΠ½Å°æ ¼Õ»ó 1¿¹¿´´Ù. °ñÀý ºÐ·ùº° ½Å°æ ¼Õ»óÀÇ ºóµµ´Â Åë°èÀûÀ¸·Î Â÷À̰¡ ¾ø¾ú´Ù. ½Å°æ ¼Õ»ó¿¡ ¿µÇâÀ» ¹ÌÄ¡´Â ¿äÀÎÀº ÈĹæÈ¯ÀÇ ÀüÀ§ Á¤µµ ¹× µ¿¹ÝÇÑ Ã´Ãß °ñÀý·Î ³ªÅ¸³µ´Ù(p=0.006, p=0.048).
°á·Ð: °ñ¹Ýȯ ¼Õ»ó ½Ã ÀÌ¿Í °ü·ÃµÈ ½Å°æ ¼Õ»óÀº 21%¿¡¼ ¹ß»ýÇÏ¿´À¸¸ç, ÈĹæÈ¯ÀÇ ÀüÀ§ Á¤µµ¿Í µ¿¹ÝµÈ ôÃß °ñÀýÀÌ ½Å°æ ¼Õ»ó¿¡ ¿µÇâÀ» ¹ÌÄ¡´Â °ÍÀ¸·Î ³ªÅ¸³µ´Ù.
Purpose: To evaluate the incidence of neurologic injury in pelvic ring injuries and to assess the risk factors for neurologic injury related to pelvic fractures.
Materials and Methods: Sixty-two patients with the pelvic ring injury were enrolled in the study from March 2010 to May 2013. When the neurologic injury was suspected clinically, the electro-diagnostic tests were performed. Combined injuries, fracture types, and longitudinal displacements were examined for correlations with the neurologic injury.
Results: There were 7 cases of AO/OTA type A, 37 cases of type B, and 18 cases of type C. Among them, 25 patients (40%) had combined spine fractures, and the average of longitudinal displacement was 7 mm (1-50 mm). Of the 62 patients, 13 (21%) had neurologic injury related with pelvic fractures; 5 with lumbosacral plexus injury, 5 with L5 or S1 nerve injury, 2 with obturator nerve injury, and 1 case of lateral femoral cutaneous nerve injury. There were no relationships between the neurologic injuries and fracture types (p=0.192), but the longitudinal displacements of posterior ring and combined spine fractures were related to the neurologic injury within pelvic ring injury (p=0.006, p=0.048).
Conclusion: The incidence of neurologic injury in pelvis fracture was 21%. In this study, the longitudinal displacements of posterior ring and combined spine fractures were risk factors for neurological injury in pelvic ring injury.
