Abstract

본 연구는 자살생각을 가진 노인들 중 자살시도에 미치는 영향요인을 파악하기 위해 2001년부터 2011년까지 전국 규모로 실시된 국민건강영양조사(Korea National Health and Nutrition Examination Survey, KNHANES)자료 중 건강 설문조사를 이용한 서술적 상관관계연구이다. 국민건강영양조사는 질병관리본부 연구윤리심의위원회의 승인을 받아 수행된 것이며, 연구대상은 65세 이상 노인 14,179명 중 자살생각이 있는 대상자 2,347명이며, 조사기간은 2001년 1월부터 2011년 12월까지이다. 통계방법은 자살생각에 유의한 차이가 있는 독립변수를 다중로지스틱회귀분석(multiple logistic regression analysis)을 실시하여 오즈비, 절단값, 예측효율성을 구하였다. Model Ⅰ는 문헌고찰에서 가장 자살시도와 관련성이 높다는 심리적 요인들을 위계적 회귀 분석한 결과 스트레스, 2주 이상 우울감, 우울증이 있는 사람이 자살시도를 할 확률이 높은 것으로 나타났으며 68.7%의 예측효율성이 나타났다. Model II는 심리적 특성과 신체적 특성을 위계적 회귀 분석한 결과 스트레스, 2주 이상 우울감, 우울증. 흡연경험이 있는 경우 자살시도를 할 확률이 높은 것으로 나타났으며, 예측효율성은 77.2%로 자살시도를 예측할 확률이 증가하여 ModelⅡ가 가장 적합하다고 할 수 있다.

This is a descriptive correlational study to determine factors affecting suicide attempts of the aged with suicide ideation by using data of a nation-wide survey, Korea National Health and Nutrition Examination Survey (KNHANES) conducted from January 2001 to December 2011. The survey was approved by Institutional Review Board of Korea Centers for Disease Control and Prevention. The subjects of this study were 2,347 elderly people with suicide ideation among 14,179 aged 65 or older. Statistically, multiple logistic regression analysis was conducted with independent variables showing a significant difference in suicidal ideation to obtain odds ratios, cutoff values, the efficiency of predicting suicide attempts. ModelⅠrevising psychological factors found to be highly related with suicide attempts in literature review, showed that the elderly experiencing a high level of stress and depression for over two weeks attempted suicide more than those with a low level of stress and without depression for over two weeks, respectively, in hierarchial regression analysis and its efficiency was 68.7%. ModelⅡ revising both of psychological and physical characteristics, revealed that the aged having a high level of stress, depression for over two weeks and depressive symptoms and having smoked attempted suicide more than those with a low level of stress and without depression for over two weeks, depressive symptoms and smoking history, respectively, in hierarchial regression analysis. As the positive and the negative predictive values and the efficiency of ModelⅡ recorded 72.8%, 77.5% and 77.2%, respectively, it is considered to be more appropriate to predict suicide attempts of elderly people.
