Abstract

¸ñ Àû: ¹æ»ç¼± Ä¡·á½Ã ÁÖÁöÀûÀ¸·Î ½ÃÇàÇÏ´Â ¸»ÃÊ Ç÷¾× °Ë»ç¿¡¼­ ¹éÇ÷±¸ ¼ººÐ Áß ¸²ÇÁ±¸¼öÀÇ °¨¼Ò°¡ °üÂûµÇ¾î, ÀúÀÚ´Â ¹æ»ç¼± Ä¡·á Àü, ÈÄÀÇ ¹éÇ÷±¸ ¼ººÐ ¹× ¸²ÇÁ±¸ ¾ÆÇü ºÐ¼®À» ½ÃµµÇÏ¿© ¹æ»ç¼± Ä¡·á°¡ °¢ ¼ººÐ¿¡ ¹ÌÄ¡´Â ¿µÇâÀÇ Á¤µµ¸¦ ¾Ë¾Æº¸°íÀÚ ÇÏ¿´´Ù.

´ë»ó ¹× ¹æ¹ý: 1994³â 12¿ùºÎÅÍ 1995³â 5¿ù±îÁö µ¿¾Æ´ëÇб³º´¿ø Ä¡·á¹æ»ç¼±°ú¿¡ ³»¿øÇÑ È¯ÀÚ Áß 16¸í(Æó¾Ï, ´ã°ü¾Ï, ½Äµµ¾Ï : 2¿¹,³ú¼Û°úü Á¾¾ç,À§¾Ï,Á÷Àå¾Ï,¾Ç¼º Èæ»öÁ¾, ¾È»ó ¹è¼¼Æ÷Á¾, ¹æ±¤¾Ï, Àü¸³¼± À°Á¾, ¼º»ó¼¼Æ÷Á¾, ´ÙÇü¼º±³¸ð¼¼Æ÷Áõ, ´Ù¹ß¼º °ñÀüÀÌ Æó¾Ï : 1¿¹)À» ´ë»óÀ¸·Î ÇÏ¿´´Ù. ¹æ»ç¼± Ä¡·á´Â 2700 cGy¿¡¼­ 6660 cGy±îÁö ½ÃÇàÇÏ¿© Á¤Áß¾Ó ÃÑ ¹æ»ç¼±·®ÀÌ 5400 cGy¿´´Ù. ¹éÇ÷±¸ ¹× °¨º°°è»ê¿¡¼­ ¹æ»ç¼± Ä¡·á Àü°ú ÈÄÀÇ ¹éÇ÷±¸ ¹× ¸²ÇÁ±¸, ´ÜÇÙ±¸, °ú¸³±¸ÀÇ Àý´ë°ª°ú ¹éºÐÀ²À» ±¸ÇÏ¿´°í ¸²ÇÁ±¸ ¾ÆÇüÀÇ ºÐ¼®Àº À¯¼¼Æ÷ºÐ¼®±â¸¦ ÀÌ¿ëÇÏ¿© ÃÑ T ¼¼Æ÷, ÃÑ B ¼¼Æ÷, Á¶·ÂÀ¯¹ß T ¼¼Æ÷, ¾ïÁ¦À¯¹ß T ¼¼Æ÷, ÀÚ¿¬»ìÇؼ¼Æ÷ µîÀÇ Àý´ë°ª°ú ¹éºÐÀ²À» ±¸ÇÏ¿´´Ù. ¹æ»ç¼± Ä¡·á ÀüÈÄÀÇ Àý´ë°ª°ú ¹éºÐÀ²À» ºñ±³ÇÏ¿´À¸¸ç Á¶·ÂÀ¯¹ßT¼¼Æ÷¿¡ ´ëÇÑ ¾ïÁ¦À¯¹ß T ¼¼Æ÷ÀÇ ºñ(Helper/Suppressor T cell ratio)ÀÇ º¯È­µµ ºÐ¼®ÇÏ¿´´Ù. ³ª¾Æ°¡ ¹æ»ç¼±·®¿¡ µû¸¥ °¢ ±¸¼ººñÀÇ º¯È­ Á¤µµ¸¦ ºÐ¼®ÇÏ¿© ÃÑ ¹æ»ç¼±·®°úÀÇ »ó°ü °ü°è¸¦ À¯ÃßÇÏ°íÀÚÇÏ¿´´Ù.

°á °ú: °¢ ȯÀÚ¿¡¼­ ¹æ»ç¼± Ä¡·á ÀüÈÄ¿¡ ÃøÁ¤ÇÑ °ªÀÇ ºñ±³¿¡¼­, ¹éÇ÷±¸¿Í ±× ±¸¼º ¼ººÐÀθ²ÇÁ±¸, ´ÜÇÙ±¸ ¹× °ú¸³±¸ÀÇ ¼ö´Â ´ÜÇÙ±¸¸¦ Á¦¿ÜÇÏ°í´Â ¹æ»ç¼± Ä¡·á Àü¿¡ ºñÇÏ¿© °¨¼ÒÇÏ¿´À¸¸ç ƯÈ÷ ¸²ÇÁ±¸ ¼öÀÇ °¨¼Ò´Â Åë°èÀûÀ¸·Î À¯ÀÇÇÑ Â÷À̸¦ º¸¿´´Ù.(p<0.05). ¸²ÇÁ±¸ ¾ÆÇüÀÎ ÃÑ T¼¼Æ÷, ÃÑ B¼¼Æ÷, Á¶·ÂÀ¯¹ß T ¼¼Æ÷, ¾ïÁ¦À¯¹ß T ¼¼Æ÷, ÀÚ¿¬»ìÇؼ¼Æ÷ ¸ðµÎ Ä¡·áÀü¿¡ ºñÇØ °¨¼ÒÇÏ¿´À¸¸ç(p¡Ã0.05), Á¶·ÂÀ¯¹ß T ¼¼Æ÷¿¡ ´ëÇÑ ¾ïÁ¦À¯¹ß T ¼¼Æ÷ÀÇ ºñ(Helper/suppressor T cell ratio)´Â ¹æ»ç¼±Ä¡·á Àü 1.52¿¡¼­ Ä¡·áÈÄ 1.11·Î °¨¼ÒÇÏ¿´°í(p<0.05) ¹æ»ç¼±·®¿¡ µû¸¥ Á¶·ÂÀ¯¹ß T ¼¼Æ÷¿¡ ´ëÇÑ ¾ïÁ¦À¯¹ß T ¼¼Æ÷ÀÇ ºñÀÇ ¹æ»ç¼± Ä¡·á ÀüÈÄ¿¡ 50 Gy ¹Ì¸¸±º(5¸í)°ú 50 Gy ÀÌ»ó±º(11¸í)¿¡¼­ °¢°¢ 0.75¿Í 0.71À̾ú´Ù.

°á ·Ð: ¹æ»ç¼± Ä¡·á Èĸ²ÇÁ±¸ ¼ö¿Í Á¶·ÂÀ¯¹ß T ¼¼Æ÷¿¡ ´ëÇÑ ¾ïÁ¦À¯¹ß T ¼¼Æ÷ÀÇ ºñ´Â °¨¼ÒÇÏ¿´°í ¾ïÁ¦À¯¹ß T ¼¼Æ÷ÀÇ ¹éºÐÀ²Àº Áõ°¡ÇÏ¿´´Ù. ÀÌ»óÀÇ °á°ú·Î ¸²ÇÁ±¸ ¾ÆÇü Áß Á¶·ÂÀ¯¹ß T ¼¼Æ÷°¡ ¾ïÁ¦À¯¹ß T ¼¼Æ÷º¸´Ù ¹æ»ç¼±¿¡ º¸´Ù ¹Î°¨ÇÑ °ÍÀ¸·Î »ç·áµÈ´Ù. º» ½ÇÇè¿¡¼­ ¹æ»ç¼±·®¿¡ µû¸¥ ¸²ÇÁ±¸ ¼ººÐÀÇ º¯È­ ºÐ¼®Àº ´ë»ó±º ¼ö°¡ Àû°í ÀÏȸ ºÐÇÒ ¹æ»ç¼±·®À̳ª Ä¡·á ºÎÀ§ÀÇ ³ÐÀÌ, ȯÀÚÀÇ Ã¼Ç¥ ¸éÀûÀÇ Â÷ÀÌ¿¡ µû¸¥ Á¦ÇÑÀû ¿ä¼Ò°¡ ÀÖ¾úÀ¸¸ç ÇâÈÄ º¸´Ù ¸¹Àº ´ë»ó±º¿¡ ´ëÇÑ ½Éµµ ±íÀº ºÐ¼®ÀÌ ¿ä±¸µÈ´Ù.
Purpose: To evaluate the changes of differential counts and lymphocyte subsets in cancer patients¡¯ leukocyte before and after radiotherapy.

Methods and Materials: From Dec. 1994 to May 1995, the changes of leukocyte and its subsets in 16 patients who received radiotherapy in the Dept. of Radiation Oncology of Dong-A University Hospiatal were investigated. Radiation was delivered from 2700 cGy to 6660 cGy with median dose of 5400 cGy. The results of pre- and post-radiotherapy were analyzed by paired T-test. The results of patients who received < 50 Gy and ¡Ã 50 Gy were analyzed by wilcoxon test.

Results: Before and after radiotherapy, there was not any significant differences in the counts of leukocyte, granulocyte and monocyte. A remarkable decrease was noted in lymphocyte counts after radiotherapy(p=0.015). T cells, B cells and natural killer cells were also decreased in number after radiotherapy but it was not significant statistically. T helper cells and T suppressor cells were also decreased in number(p>0.05). The ratio of T helper/suppressor cell was decreased from 1.52 to 1.11 and it was significant statistically(p=0.016). The portion of T suppressor cell among all T cells was increased after radiotherapy (p=0.0195). No significant difference was observed in the analysis of leukocyte and its subsets between patients who reveived < 50 Gy and ¡Ã 50 Gy.

Conclusion: Radiotherapy caused remarkable decrease in lymphocyte count and its subsets. Among all lymphocyte subsets, T helper cell might be the most vulnerable to radiation, considering decreased ratio of T helper/surppressor cell count after radiotherapy.
