Abstract

목적 : 극소 저체중 출생아는 퇴원 후에도 후유증 및 정상아와 비교하여 차이 있는 성장과 발달 형태로 양육부담과 불안이 커질 수 있어서, 지속적인 육아상담과 정보제공, 상호교류 등이 필요하다. 저자들은 장해를 초 조기에 찾아내는 육아 접근보다는 가족지원에 무게를 두고 관련 직종이 함께 상담하며 1년간 육아교실을 운영하였다. 육아 불안해소와 성장 및 발달을 촉진하고 합병증의 재택치료 방법을 교육시키면서 경도의 장해발견과 대응, 부모의 자기 평가 회복과 모자간의 건강한 애착관계 형성, 부모의 자조 그룹 형성 등을 육아 교실 운영의 목표로 하였다.

방법 : 교정 연령 6개월 미만의 출생체중 1,500 g 미만의 극소 저체중 출생아를 대상으로 하였다. 1년간 월 1회 모임을 가졌다. 육아지원 교실은 놀이프로그램을 계획하여 매회 음악치료사와 함께 노래 및 율동을 배우는 시간을 가졌고, 육아교육 프로그램으로 신생아의사와 아동전문 간호사, 육아 지도자, 임상심리사와 재활치료사, 사회복지사, 영양사로 이루어진 각 분야 전문가 등의 강의를 듣는 시간을 가졌다. 경험담 나누기는 가족간의 대화를 통해 육아에 관련된 고민을 말하고 정보를 교환하여 모친들의 자조모임을 구성할 수 있도록 하였다. 가정 방문을 시행하여 산소투여, 경관급식, 뇌성마비 의심 등을 관찰하였으며 방임이나 학대가 의심되는 경우에 2차 방문을 하였다.

결과 : 참석한 극소 저체중 출생아는 51명이었다. 출생 체중은 평균 1,060 g, 임신 재태기간은 평균 27주 2일이었고, 성별은 남아 18명, 여아 23명이었다. 모친 평균 연령은 33.4세였으며 적정 체중아 26명, 부당 경량아 8명, 부당 중량아 5명이었다. 출생 순서는 첫째 23명, 둘째가 13명, 셋째가 1명, 쌍생아 4쌍이었다. 가정방문은 1차 방문 33명, 2차 방문 6명이었다. 참석군에서 모친의 우울 지수가 10.47± 5.18에서 8.18±5.87로 줄었다(P=0.080).

결론 : 육아 교실에 참석한 모친의 우울지수가 줄었다.

Purpose : After discharge from the hospital, very low birth weight infants require care consultation, information, and interaction and so forth, due to sequelae, different growth and developmental pattern; which increase the burdens and the worries of patients in nurturing them. With the counseling of experts in the related fields, the authors and the Food for the Hungry have run a department of childcare for a year, emphasizing on more family support than extreme early detection of disabilities. This study reports on relieving child care anxiety, promoting growth and development, educating parents on home treatment of sequelae, finding and coping with minor disabilities.

Methods : The subjects were chosen among infants under 1,500 g of birth weight and younger than a corrected age of 6 months. Meetings were held once in a month for a year. Infant care support classes were taught by experts in their fields including a music therapist (with play program), neonatologist, a children’s nurse, a child-care director, a clinical psychologist, a rehabilitation therapist, a social worker and, a nutritionist. Within the self-dependent groups, the families were able to share their concerns and experiences on child-caring. Second home visits were carried out to monitor home oxygen therapy, tube feeding, cerebral palsy signs, and to find neglect or abuse in suspected cases.
Results : Fifty-one infants participated; the average weight was 1,060 g, the average gestational age was 27 weeks and 2 days. Eighteen were boys and 23 girls. The average age of the mothers was 33.4, and there were 26 appropriate for gestational age, 8 small for gestational age, and, 5 large for gestational age. Twenty three were first-born, 13 were second-born, 1 was third-born, and twins were 4 pairs. Home visits were done in 33 families, and secondary visits were done in 6 families. The depression score of mothers in the attendant group decreased from 10.47±5.18 to 8.18±5.87 (P=0.080).

Conclusion : The depression score of mothers in the attendant group decreased after infant care support classes.
