Abstract

목적: 해리수직편위 환자에서 편위량이 큰 눈이 주시안인 경우가 실제 임상에서 어느 정도 비율을 차지하는지 알아보고자 하였다.

대상과 방법: 2001년 1월부터 2011년 11월까지 본원 안과에서 해리수직편위를 진단받은 환자를 대상으로 의무기록을 통하여 후향적으로 조사하였다.

결과: 총 71명 중 편위량이 큰 눈이 주시안인 경우는 25명으로 35.21%에 해당하였고, 비주시안인 경우도 26명으로 36.62%에 해당하였다. 나머지 21명은 29.58%로 편위량이 큰 눈이 주시안인지 비주시안인지 뚜렷이 정할 수 없었다. 해리수직편위를 교정하기 위해 양안 상직근후전술 또는 하사근전치술을 시행 받은 환자는 주시안의 편위량이 더 큰 경우가 25명 중 20명이었고 비주시안의 편위량이 더 컸던 경우는 26명 중 22명이었다.

결론: 해리수직편위의 편위각이 큰 눈이 주시안인 경우가 적지 않은 비율을 차지하므로 해리수직편위 환자를 진료함에 있어 편위량의 측정뿐 아니라 어느 눈이 주시안인지를 알아내는 것이 수술적 처치 등의 치료 방향에 도움을 줄 수 있을 것으로 생각한다.

Purpose: To clinically determine the proportion of people with a fixed dissociating eye in patients with dissociated vertical deviation (DVD).

Methods: Medical records of 86 patients diagnosed with DVD in our department of ophthalmology from 2001 to November 2011 were retrospectively analyzed.

Results: Of the 71 patients, 25 (35.21%) showed a higher degree of dissociation in the fixating eye, 26 (36.62%) had higher dissociation in the non-fixating eye; 20 of 25 patients with a higher level of dissociation in the fixating eye and 22 of 26 patients with a higher level of dissociation in the non-fixating eye received surgical correction for DVD, such as superior rectus muscle recession or inferior oblique muscle anterior transposition.

Conclusions: In DVD, the proportion of patients showing a higher level of dissociation in the fixating eye was significant. Patients with DVD require thorough evaluation; measurement of the level of dissociation and determination of the fixating eye is beneficial for management and surgical intervention.
