Abstract

목적: 소아에서 발병한 단순포진바이러스에 의한 일차성 감염에 의해 면역성 가장자리 각막궤양을 유발한 증례를 경험하고 치료하였 기에 이를 보고하고자 한다.

증례요약: 7세 환아가 5일 전부터 발생한 좌측 눈꺼풀의 수포성 병변을 동반한 부종과 안구의 통증 및 시력저하로 내원하였다. 세극등 현미경 검사에서 좌안의 결막 충혈과 함께 상측 주변부 각막에 상피결손이 관찰되었다. 상피결손 주위로 각막의 기질침윤과 눈꺼풀결 막의 위막소견이 관찰되었다. 좌안의 각막지각은 우안에 비해 감소되어 있었다. 저자들은 단순포진바이러스의 일차성 감염으로 진단 하고, 항바이러스제를 점안 및 경구로 투여하였으며 부신피질호르몬제를 점안하였다. 치료 5일 후 각막의 병변은 감소하였으며 눈꺼 풀의 수포성 병변은 가피형성 후 소실되었다. 치료 6개월 후 경도의 각막 혼탁 소견을 보였으나 그 이외 합병증은 병발하지 않았다.

결론: 저자들은 단순포진바이러스의 일차성 감염으로 국내 보고되지 않은 가장자리 각막궤양을 경험하였고, 항바이러스제와 스테로이 드 점안제로 치료한 1예를 보고한다.

Purpose: To report a case of peripheral ulcerative keratitis in a case of primary herpes simplex infection.

Case summary: A 7-year-old male complaining of pain, redness, and decreased vision in the left eye 5 days in duration visited our clinic. The patient had also vesicular lesions over the edematous left upper eyelid. Slit-lamp examination revealed peripheral ulcerative keratitis with stromal infiltration involving the superior peripheral cornea. Corneal sensation in the left eye was decreased. The patient was advised to use topical and oral antiviral agents and a topical steroid. After 5 days of follow-up, the corneal lesions were decreased and vesicles were absent. Six months later, only minor opacities remained in the cornea.

Conclusions: We report the first case report in Korea of peripheral ulcerative keratitis with blepharoconjunctivitis presented as primary herpes simplex infection which responded to antiviral and steroid treatment in a young male.
