Abstract

목적: 운동축을 삽입한 하이드록시아파타이트 안와삽입물에서 발생한 방선균 감염 2예를 경험하였기에 이를 보고하고자 한다.

증례요약: 17년 전 좌안 안구내용제거술 및 하이드록시아파타이트 안와삽입물 삽입술을 시행 받은 44세 남자 환자와 15년 전 좌안 안구제거술 및 하이드록시아파타이트 안와삽입물 삽입술을 받은 55세 남자 환자가 1년여 전부터 결막낭에 출혈이 발생하고 살이 차오 르는 증상으로 내원하였다. 두 환자 모두에서 수술 후 운동축 삽입술을 시행 받은 병력이 있었으며, 내원 당시 결막낭에 육아종이 관찰되었고 육아종 제거 시에 안와삽입물이 노출된 소견을 보였다. 안와삽입물 제거술 및 자가 진피지방 이식술을 시행하였고 조직병 리검사 결과 방선균에 의한 안와삽입물 감염 소견이 확인되었다.

결론: 다공성 안와삽입물을 가진 환자에서 결막의 만성 화농성 육아종과 안와삽입물의 노출이 지속되는 경우, 방선균 감염의 가능성을 고려하여야 한다. 방선균 감염은 드문 감염 질환으로, 하이드록시아파타이트 안와삽입물에서의 감염은 현재까지 국내에 보고된 예가 없어 이를 보고하는 바이다.

Purpose: To report 2 cases of Actinomyces infection in a hydroxyapatite orbital implant with a motility peg.

Case summary: A 44-year-old male and a 55-year-old male who underwent evisceration and implantation of a hydroxyapatite implant in the left eye 17 and 15 years prior, respectively, presented with a conjunctival sac granuloma with discharge and bleeding of 1 year duration. Both patients had a history of motility peg implantation. A large-area of the hydroxyapatite implant was exposed after removal of the granuloma. The previous orbital implant was removed, and the exposed area was covered with a dermis fat graft in both patients. On histopathological examination, Actinomyces infection in the orbital implant was observed in both patients.

Conclusions: To the best of our knowledge, this is the first case report of actinomycosis of hydroxyapatite orbital implant in Korea. In a patient with a porous orbital implant, the possibility of Actinomyces infection of the orbital implant should be considered after a long-duration and large-area exposure of the implant.
