양안의 비측 상안검에 발생한 특발성 경화성 염증성 종양 1예

Abstract

목적: 통증을 동반한 양안의 비측 상안검 종괴를 주소로 내원한 환자에서 특발성 경화성 염증성 종양으로 진단된 1예를 경험하였기에 보고하고자 한다.

증례요약: 26세 여자가 3-4개월 전부터 시작된 상측 주시 시의 양안 통증과 양안의 비측 상안검에서 점점 크기가 커지는 종괴를 주소 로 내원하였다. 안와 자기공명영상촬영에서 양성 종양으로 의심되어 상대적으로 큰 우안의 종괴에 대하여 부분 절제 생검을 시행했으 며 섬유화를 동반한 만성적인 염증소견을 보였다. 수술 후 11일 동안 경구 스테로이드를 투여하였으나 종괴의 크기 변화는 없었고 일차 수술 6주 후 양안 종괴의 바닥까지 완전 절제생검을 시행하였으며 특발성 경화성 염증성 종양으로 진단하였다. 스테로이드 정맥 주사를 일주일 간격으로 두 차례 처방하였고 수술 후 6개월까지 경과 관찰하였으나 재발의 소견은 보이지 않았다.

결론: 특발성 경화성 염증성 종양은 대개 단안에서 안와의 앞쪽, 바깥쪽 및 안와 첨부에 발생하며 하비측은 침범하지 않고 만성적인 경과를 보이는 것으로 알려졌으나 드물게 양안의 비측 상안검에 발생하여 완전 절제술로 치료한 1예가 있어 보고하는 바이다.

Purpose: To report a case of idiopathic sclerosing inflammatory tumor presenting as painful masses in the medial side of both upper eyelids.

Case summary: A 26-year-old female presented with pain in both eyes at upward gaze and progressive medial side masses on bilateral upper eyelids 3-4 months prior. Orbit MRI showed an orbital benign tumor and partial excisional biopsy was performed for the larger right upper eyelid mass. The biopsy result was chronic inflammation with fibrosis. There was no change in the masses size after an oral steroid was prescribed for 11 days. At 6 weeks after the first operation, complete excisional biopsy was performed for the bilateral upper eyelid masses and idiopathic sclerosing inflammatory tumor was diagnosed. Intravenous steroid injections were administered twice with a 1-week interval postoperatively. After 6 months of follow-up, no recurrence was evident.

Conclusions: Idiopathic sclerosing inflammatory tumor usually involves the anterior, lateral, or apex of the unilateral orbit and does not invade the inferomedial side of the orbit and typically has a chronic course. We experienced a rare case of idiopathic sclerosing inflammatory tumor that involved the medial side of both upper eyelids which was cured by complete excision.
