Abstract

목적: 아래눈꺼풀의 무통성 종괴로 안과를 방문한 환자의 종괴 조직검사에서 눈꺼풀에서 발생한 원발성 반지세포암종으로 진단된 드문 증례를 경험하고 이를 보고하고자 한다.

증례요약: 74세 남자가 7개월 전부터 발생하여 서서히 커지는 우측 아래눈꺼풀의 무통성 종괴를 주소로 내원하였다. 종괴 및 종괴가 위치하는 아래눈꺼풀 피부의 생검술을 시행하였으며, 조직검사에서 반지세포암종 소견을 보였다. 추가로 시행한 전신 양전자방출 단층촬영(PET-CT), 흉부 전산화단층촬영, 복부 및 골반 전산화단촬영, 위장관 및 결장 내시경검사 등과 같은 전신 검사에서 이상 소견을 발견할 수 없었다. 이에 눈꺼풀에 발생한 원발성 반지세포암종으로 진단하고 7주 동안 6600 cGy 단위로 총 33회 우측 눈꺼풀에 방사선 치료를 시행하였다.

결론: 저자들은 방사선치료 후 5년간 경과 관찰하는 동안 재발이나 전이가 없었던 눈꺼풀에 발생한 원발성 반지세포암종을 경험하였기에 보고하고자 한다.

Purpose: To report a rare case of a painless mass on the lower lid, histologically diagnosed as primary signet ring cell carcinoma of the eyelid.

Case summary: A 74-year-old male presented with a painless mass on the right lower lid which had developed seven months prior to presentation. Incisional biopsy of the mass and attached lower lid skin was performed, revealing signet ring cell carcinoma. A systemic evaluation, including whole body positron emission tomography-computed tomography (PET-CT), chest CT, abdomen and pelvis CT, gastrointestinal endoscopy, and colonoscopy, revealed no other abnormal lesions. Therefore, the eyelid lesion was considered primary signet ring cell carcinoma of the skin and was treated with radiotherapy of 6600 cGy in 33 fractions over 7 weeks.

Conclusions: Herein, the authors report a rare case of primary signet ring cell carcinoma of the eyelid with no evidence of tumor recurrence or metastasis for 5 years after radiotherapy.
