Abstract

목적: 유리체절제술과 백내장 동시수술 시 모양체고랑에 인공수정체를 삽입하였을 때 수술 전 굴절이상예측값과 수술 후 현성굴절이 상값의 차이를 알아보았다.

대상과 방법: 유리체절제술과 백내장 동시수술 시 인공수정체를 모양체고랑에 삽입했던 환자와 인공수정체를 수정체낭에 삽입했던 환자들의 수술 전 굴절이상예측값과 수술 후 현성굴절이상값을 후향적으로 조사하였다.

결과: 유리체절제술과 백내장 동시수술 시 인공수정체를 모양체고랑에 삽입했던 제1군은 19명 19안이었고, 인공수정체를 수정체낭에 삽입했던 제2군은 20명 23안이었다. 수술 후 현성굴절이상값과 수술 전 굴절이상예측값의 차이는 제1군에서는 -0.71 ± 1.02D였고 (p=0.012), 제2군에서는 -0.07 ± 0.74D (p=0.86)로, 제1군의 수술 후 현성굴절이상값은 인공수정체를 수정체낭에 넣은 제2군과 비교해 유의한 근시편위를 보였다.

결론: 유리체절제술과 백내장 동시수술 시 인공수정체를 모양체고랑에 삽입할 경우, 인공수정체 도수를 약 1.0D 낮추어 인공수정체를 삽입하는 것이 수술 후 원하는 굴절력을 얻는 데 도움이 될 것으로 생각된다.

Purpose: To evaluate the difference between the predicted postoperative refraction and the actual postoperative refraction after simultaneous vitrectomy and cataract surgery with sulcus fixation of an intraocular lens (IOL).

Methods: The records of patients with simultaneous vitrectomy and cataract surgery with sulcus IOL implantation (group 1) and with in-the-bag IOL implantation (group 2) were reviewed. We compared the spread between the predicted and actual refractive errors in the two groups.

Results: Group 1 consisted of 19 eyes in 19 patients and group 2 consisted of 23 eyes in 20 patients. The spread between the predicted and actual refraction (spread = actual-predicted) was -0.71 ± 1.02 D in group 1 (p = 0.012) and -0.07 ± 0.74 D in the group 2 (p = 0.86). The actual refractive errors in group 1 shifted toward myopia from the predicted in-the-bag refraction when compared with group 2.

Conclusions: For the implantation of an IOL in the sulcus after simultaneous vitrectomy and cataract surgery, the IOL power should be approximately 1.0 D less than the power for in-the-bag fixation.
