Abstract

목적: 데모덱스 안검염 환자에서 Polyhexamethylene Biguanide (PHMB)를 이용한 안검 세척의 치료효과에 대해서 알아보고자 한다.

대상과 방법: 데모덱스 안검염으로 진단된 환자 31명을 대상으로 0.4% PHMB를 이용하여 하루 2회씩 총 8주간 안검 세척을 시행하였고, 2주 간격으로 치료효과를 평가하였다. 내원 시마다, 환자의 속눈썹을 채취한 후, 광학 현미경으로 관찰된 데모덱스 수를 기록하였고, 세극등 현미경 검사를 통해 눈물막 파괴시간을 측정하였다. 주관적 증상에 대한 지표로는 안구표면질환지수(OSDI)를 이용하였다. 또한 환자의 순응도를 3단계로 나누어 기록하여, 순응도 차이에 따른 오류를 보정하였다.

결과: 순응도가 좋지 않았던 1명을 제외하였다. 8주간 안검 세척을 시행한 후, 데모덱스 안검염 환자 30명 중 28명에서 데모덱스 수가 감소하였으며, 시행 전 평균 7.9 ± 3.6마리에서 시행 후 2.2 ± 2.4마리로 감소하였다(p＜0.01). 눈물막 파괴시간은 시행 전 평균 2.7 ± 0.8초에서 시행 후 3.4 ± 0.9초로 증가하였다(p＜0.01). 또한 30명 중 28명에서 안검 세척을 시행한 후 안구표면질환지수가 시행 전 평균 22.0 ± 10.7점에서 시행 후 7.4 ± 6.0점으로 감소하였다(p＜0.01).

결론: 데모덱스 안검염의 치료에 있어 PHMB를 이용한 안검 세척이 데모덱스 수를 감소시키고, 눈물막 파괴시간을 증가시키며, 안구표면 질환지수를 감소시키는 데 효과가 있다.

Purpose: To evaluate the treatment efficacy of polyhexamethylene biguanide (PHMB) lid scrub on Demodex blepharitis.

Methods: Thirty-one patients diagnosed with Demodex blepharitis were evaluated every 2 weeks during 8 weeks of lid scrub treatment with 0.4% PHMB. Patients underwent epilation of 4 eyelashes in each eye, and the number of Demodex lesions was counted. The patients answered questionnaires regarding ocular surface discomfort and underwent ophthalmologic exams including slit lamp and tear breakup time (TBUT). Compliance was recorded as 1 of 3 stages (good, moderate, poor).

Results: One patient was excluded for poor compliance. After PHMB lid scrub for 8 weeks, Demodex count was reduced in 28 of 30 patients (pre-PHMB 7.9 ± 3.6 counts, post-PHMB 2.2 ± 2.4 counts, p < 0.01). In addition, TBUT showed a statistically significant increase after PHMB lid scrub (pre-PHMB 2.7 ± 0.8 seconds, post-PHMB 3.4 ± 0.9 seconds, p < 0.01). Ocular surface disease index (OSDI) score was reduced in 28 of 30 patients (pre-PHMB 22.0 ± 10.7 points, post-PHMB 7.4 ± 6.0 points, p < 0.01).

Conclusions: Eight weeks of treatment with polyhexamethylene biguanide lid scrub on Demodex blepharitis had good treatment efficacy for reducing Demodex counts and OSDI scores and increasing TBUT.
