Abstract

목 적: 기존의 치료에 불응하는 상윤부 각결막염 환자에서 베바시주맙과 트리암시놀론 혼합제 주사를 이용하여 치료한 2예를 보고하고자 한다.

증례요약: 69세 여자 환자가 3일 전부터 시작된 좌안의 통증으로 내원하여 상윤부 각결막염 진단 받고 3개월간 스테로이드 점안제, 인공눈물, 경구 스테로이드를 단계적으로 사용하였으나 치료되지 않아 기존 치료에 불응한다는 판단 하에 베바시주맙 0.15 cc, 트리암 시놀론 0.05 cc을 혼합한 후 테논낭 하에 주사하였다. 주사 후 1주일 뒤 경과 관찰에서 증상 및 이학적 검사 소견 모두에서 호전을 보였고 이후 6개월간 재발이 없었다. 55세 여자 환자가 3개월 전 좌안 상윤부 각결막염 진단하 인공눈물, 스테로이드 점안제, 눈물점 폐쇄, 보톡스 치료를 단계적으로 시행하였으나 호전 없어 전원되었다. 베바시주맙 0.15 cc, 트리암시놀론 0.05 cc를 혼합하여 테논낭하에 주사한 후 2주일 뒤 증상과 이학적 검사에서 호전을 보였고 4개월간 재발이 없었다.

결 론: 기존의 치료에 불응하는 상윤부 각결막염에서 베바시주맙과 트리암시놀론 혼합제를 테논낭하에 주사하여 혈관 신생과 염증을 함께 억제하며 좋은 치료 효과를 보인 첫 증례들이므로 의미 있는 것으로 생각된다.

Purpose: To report two cases of intractable superior limbic keratoconjunctivitis (SLK) treated with bevacizumab and triamcinolone
injection.

Case summary: A 69-year-old female visited our clinic with pain in the left eye for 3 days and was diagnosed with SLK in her left eye. After 3 months of using steroid eye drops, artificial tears, and oral steroid intermittently, there was no improvement in symptoms and signs, thus this case was considered intractable with the conventional therapy. A mixture of bevacizumab (0.15 cc) and triamcinolone (0.05 cc) was injected into the sub-tenon’s capsule of the left eye. After 1 week, all
symptoms and signs disappeared, and there was no recurrence for 6 months. A 55-year-old female was transferred to our clinic due to SLK that did not respond to artificial tears, steroid eye drops, punctal occlusion, and botox injection for 3 months. A mixture of bevacizumab (0.15 cc) and triamcinolone (0.05 cc) was injected into the sub-tenon’s capsule of the left eye. After 2 weeks, all symptoms and signs were improved, and there was no recurrence for 4 months.

Conclusions: The presented 2 SLK cases are meaningful, because neovascularization disappeared and controlled inflammation was obtained following sub-tenon injection with both bevacizumab and triamcinolone.
