Abstract

¸ñÀû: Çѱ¹ÀÎ ¿¬¼Ò¼º°³¹æ°¢³ì³»Àå ȯÀÚ¿¡¼­ ¸¶ÀÌÅ丶À̽ŠC¸¦ ÀÌ¿ëÇÑ ¼¶À¯ÁÖÀýÁ¦¼úÀÇ ¼º°ø·ü°ú ÀÌ¿¡ ¿µÇâÀ» ¹ÌÄ¡´Â ÀÎÀÚ¿¡ ´ëÇÏ¿© ¾Ë¾Æº¸¾Ò´Ù.

´ë»ó°ú ¹æ¹ý: 2004³â 1¿ùºÎÅÍ 2013³â 1¿ù±îÁö ¼¶À¯ÁÖÀýÁ¦¼úÀ» ½ÃÇà¹ÞÀº ¿¬¼Ò¼º³ì³»Àå ȯÀÚ 29¸í 29¾ÈÀÇ Àǹ«±â·ÏÀ» ÈÄÇâÀûÀ¸·Î ºÐ¼®ÇÏ ¿´´Ù. ¼ö¼ú Àü°ú ¼ú ÈÄ 1ÁÖ, 1, 3, 6, 12°³¿ù°¿Í 18°³¿ù ÀÌÈÄ ¸¶Áö¸· ¹æ¹®½ÃÀÇ ¾È¾ÐÀ» ±â·ÏÇϰí, ÇÕº´Áõ ¹ß»ý ¿©ºÎ¸¦ Á¶»çÇÏ¿´´Ù. ¼ö¼ú ¼º°øÀº ¾à¹°»ç¿ë ¿©ºÎ¿Í °ü°è ¾øÀÌ ¾È¾ÐÀÌ 21 mmHg ÀÌÇϰųª ¼ö¼ú Àü ¾È¾Ðº¸´Ù 20% ÀÌ»ó Çϰ­ÇÑ °æ¿ì·Î Á¤ÀÇÇÏ¿´´Ù. ¼ö¼ú½ÇÆÐ¿Í °ü·ÃµÈ À§ÇèÀÎÀÚ¸¦ Cox ºñ·ÊÀ§Çèȸ±ÍºÐ¼®À¸·Î ¾Ë¾Æº¸¾Ò´Ù.

°á°ú: Æò±Õ¿¬·ÉÀº 24.6 (¡¾ 8.9)¼¼¿´°í, Æò±Õ ÃßÀû°üÂû ±â°£Àº 24.3 (¡¾ 20.8)°³¿ùÀ̾ú´Ù. ÃÑ ÃßÀû°üÂû ±â°£µ¿¾È Àü¹ÝÀûÀÎ ¼ö¼úÀÇ ¼º°ø·ü Àº 82.8%¿´°í, Ä«Çöõ-¸¶ÀÌ¾î »ýÁ¸ºÐ¼®¹ýÀ» ÅëÇØ ±¸ÇÑ ¼ö¼úÀÇ ´©Àû¼º°ø·üÀº 1³â° 100%, 2³â° 87.4% 3³â° 78.7%À̾ú´Ù. °á·Ð: Çѱ¹ÀÎ ¿¬¼Ò¼º°³¹æ°¢³ì³»Àå¿¡¼­ ¸¶ÀÌÅ丶À̽ŠC¸¦ ÀÌ¿ëÇÑ ¼¶À¯ÁÖ ÀýÁ¦¼úÀº È¿°úÀûÀÎ Ä¡·á¹æ¹ýÀ¸·Î »ý°¢µÈ´Ù.
Purpose: To evaluate the outcomes and prognostic factors of trabeculectomy with mitomycin C in Korean patients with juvenile open-angle glaucoma (JOAG).

Methods: A retrospective review was performed on 29 eyes with JOAG who had undergone trabeculectomy between January 2004 and January 2013. Intraocular pressure (IOP) and postoperative complications were monitored at 1 day preoperatively, at 1 day, 1 week, 1 month, 3 months, 6 months, and 12 months postoperatively, and at final follow-up after postoperative 18 months. Surgical success was defined as a final IOP of <21 mm Hg or <80% of preoperative IOP, regardless of the use of anti-glaucoma medication. Prognostic factors for surgical success or failure were analyzed by the Cox proportional hazards model.

Results: Mean (¡¾ standard deviation) age at trabeculectomy was 24.6 (¡¾ 8.9) years, and mean follow-up period was 24.3 (¡¾ 20.8) months. The overall success rate was 82.8% at final follow-up. The Kaplan-Meier cumulative probabilities of surgical success were 100% at 1 year, 87.4% at 2 years, and 78.7% at 3 years postoperatively. The Cox proportional hazards model failed to determine any significant factors that were associated with surgical failure. The three most frequent postoperative complications were immediately postoperative hypotony (n = 19, 65.5%), hypotony maculopathy (n = 8, 27.6%), and encapsulated bleb (n = 8, 27.6%), most of which were resolved after a minor procedure or observation. Two of 8 eyes with encapsulated bleb required a second surgery. There were no vision-threatening severe complications.

Conclusions: Trabeculectomy with MMC may be an effective treatment in Korean JOAG patients.
