Abstract
¸ñÀû: À§Àå°üÃâÇ÷·Î ÀÎÇÑ ±Þ¼º ½ÇÇ÷ ÈÄ ¾çÃø¼º ¾ÕÇãÇ÷½Ã½Å°æº´ÁõÀÌ ¹ß»ýÇÑ È¯ÀÚ 1¿¹¸¦ º¸°íÇϰíÀÚ ÇÑ´Ù.
Áõ·Ê¿ä¾à: Á¦1Çü ´ç´¢¿Í ¾ËÄݼº °£°æº¯ÀÇ º´·ÂÀÌ ÀÖ´Â 50¼¼ ³²ÀÚ È¯ÀÚ°¡ ³»¿ø 3ÀÏ ÀüºÎÅÍÀÇ Èæ»öº¯°ú ³»¿ø 1ÀÏ ÀüºÎÅÍÀÇ ¾îÁö·¯¿òÀ» ÁÖ¼Ò·Î ³»¿øÇÏ¿´´Ù. ÃÊÁø½Ã Ç÷»ö¼Ò ¼öÄ¡´Â 4.7 g/dLÀ̾úÀ¸¸ç Ç÷¾ÐÀº 100/55 mmHg¿´´Ù. »óºÎÀ§Àå°ü ³»½Ã°æ°Ë»ç»ó ÃâÇ÷ÀÌ µÇ°í ÀÖ´Â À§±Ë¾çÀÌ °üÂûµÇ¾î ÁöÇ÷°ú ¼öÇ÷ ÈÄ Àü½Å»óÅ´ ȣÀüµÇ¾úÀ¸³ª ÀÌÈÄ ¾ç¾È ½Ã·ÂÀúÇϸ¦ È£¼ÒÇÏ¿´°í ÀÌ´Â Èæ»öº¯ÀÌ ¹ß»ýÇÑ Á÷ÈĶó°í º¸°í ÇÏ¿´´Ù. °á·Ð: ÀϹÝÀûÀ¸·Î °æÇèÇÏ´Â °æ¿ì¿Í ´Þ¸® ±Þ¼º ½ÇÇ÷ ÈÄ ¾ÕÇãÇ÷½Ã½Å°æº´ÁõÀÌ µ¿¹ÝµÉ ¼ö ÀÖÀ½À» À¯ÀÇÇϰí, ƯÈ÷ ÀÌ·¯ÇÑ °æ¿ì ¾çÃø¿¡ ¹ß»ý ÇÒ ¼ö ÀÖÀ½À» °í·ÁÇØ¾ß ÇϰڴÙ.
Purpose: To introduce a case of bilateral anterior ischemic optic neuropathy (AION) after blood loss due to gastrointestinal bleeding.
Case summary: A 50-year-old male patient with a history of type 1 diabetes mellitus and alcoholic liver cirrhosis presented with 3 days of melena and 1 day of general weakness and dizziness. Initial hemoglobin level was 4.7 g/dL and blood pressure was 100/55 mm Hg. On esophagogastroduodenoscopy, a peptic ulcer with evident recent bleeding was observed. After transfusion of packed red blood cells and endoscopic hemostasis of bleeding, his general condition improved but he complained of blurred vision in both eyes which developed immediately after the onset of melena. Initial best-corrected visual acuity (BCVA) was 0.5 in his right eye and 0.6 in the left eye. On fundus examination, swollen optic disc with blurred margin was noted and he had constricted visual fields. On follow-up, the patient received posterior subtenon triamcinolone injection in his right eye. After the procedure, the BCVA was improved to 0.8 in both eyes, but he still had bilateral pale optic disc with constricted visual field.
Conclusions: In the case of visual loss after recent blood loss, AION should be considered as a diagnosis, which can present as bilateral involvement.
