Abstract

목적: 발진성 연모 낭종은 무증상의 작은 구진과 결절이 다발성으로 가슴이나 겨드랑이, 팔다리에 발생하는 양성 종양으로 소아나 청년 에서 주로 발생하며 선천성이나 중년층에서 발생하는 것은 드문 것으로 알려졌다. 저자들은 중년 남성의 눈꺼풀에 단독으로 발생한 발진성 연모 낭종 1예를 경험하였기에 문헌 고찰과 함께 이를 보고하고자 한다.

증례요약: 44세 남환이 2년전부터 서서히 자라는 윗눈꺼풀에 만져지는 무증상의 종물을 주소로 내원하였다. 촉진 시 무통성의 딱딱하고 둥근 종물이 촉지되었다. 안와 전산화단층촬영 결과 오른쪽 윗눈꺼풀 7.0×9.0×9.5 mm 크기의 종물이 관찰되어 눈썹밑 절개를 통한 종양 절제생검을 시행하였다. 병리조직학적 검사상 박판상의 각화성 물질과 연모를 포함하여 편평상피세포로 둘러싸여 있는 약 7.0×6.0×4.0 mm 크기의 낭종으로 관찰되어 발진성 연모 낭종으로 최종 진단되었으며 술 후 재발이나 추가적인 치료는 없었다.

결론: 저자들은 일반적으로 체간부 및 사지에 다발성으로 발생하는 전형적인 발진성 연모 낭종과는 다르게 비교적 드문 부위인 눈꺼풀 에 단독으로 발생한 비전형적인 발진성 연모 낭종 1예를 경험하였기에 이를 보고하며 눈꺼풀 종양의 감별 진단 시 염두해 두어야 할 것으로 생각한다.

Purpose: Eruptive vellus hair cysts (EVHC) are benign lesions that affect the pediatric population and are rarely seen congenitally or in young adults. EVHCs are small, cystic papules that usually occur on the chest and proximal extremities. EVHCs of the eyelids have been reported infrequently. We experienced a case of solitary EVHC that developed on the eyelid in a middle-aged male. Herein, we present our case with a brief review of the literature.

Case summary: A 44-year-old male presented with a history of an asymptomatic, palpable mass in the right upper eyelid that had been slowly growing for 2 years. Physical examination revealed non-tender, firm and round mass in right upper eyelid. Computed tomography scan of the orbit showed a 7.0 × 9.0 × 9.5 mm-sized focal bulging contour in the right upper eyelid. The patient underwent sub-brow incision and excisional biopsy of the eyelid mass. Histopathological examination revealed a 7.0 × 6.0 × 4.0 mm-sized cystic structure lined by squamous epithelium containing laminated keratinous materials and multiple vellus hair shafts. There was no local recurrence during the postoperative follow-up.

Conclusions: We experienced a rare case of EVHC that developed on the eyelid. Based on our experience, EVHC should be considered when determining an eyelid mass, especially around hair follicles.
